Job summary
We have an exciting opportunity for a motivated and experienced Clinical Pharmacist to join our established Primary Care Network team working across our sites. This role offers the chance to make a real impact on patient care, supporting GP practices to deliver safe, effective and patient-centred medicines management.
Working as part of a supportive multidisciplinary team, you will use your clinical expertise to improve outcomes for patients, particularly those with long-term conditions and complex medication needs. You will play a key role in optimising prescribing, reducing medicines-related risks and supporting patients to get the best from their treatment.
This is a part-time role (26 hours per week) with working pattern to be agreed, offering an excellent opportunity to develop your clinical skills within a forward-thinking and collaborative Primary Care environment.
Main duties of the job
The post holder will work as part of the PCN multidisciplinary team to support medicines optimisation and deliver high-quality patient care.

Undertaking structured medication reviews for patients, particularly those with complex polypharmacy and long-term conditions
