Enfield Unity Primary Care Network is the second largest PCN in England representing over 158,000 patients across 18 GP Practice sites. Our aim is to improve services and outcomes for patients by supplying a well-trained, motivated workforce.
We are seeking a qualified Diabetes Specialist Nurse Practitioner (DSN) to join our team. The aim of the service is to improve diabetes care at all levels and ensure all those living with diabetes are known to our services to reduce their likelihood of developing complications and improve their ability to manage their own wellbeing, ensuring integration of streamline processes, reduced duplication, improved communication and delivery of cost-effective, evidence-based, patient-focused diabetes care.
Main duties of the job
The role of the Diabetes Specialist Nurse (DSN) is to educate the wider clinical team to promote the health and wellbeing of people with diabetes enabling them to achieve optimum Glycaemic, Blood Pressure, Cholesterol and other key parameters.
The DSN will offer diabetes service at various sites, see patients with complex diabetes problems (Tier 3) as well as delivering an education programme to our wider clinical team of GPs, Clinical Pharmacist and other HCPs.
The DSN will run joint clinics with the wider team and will take part in MDT meetings at site level.
The post holder will work with local community providers, PCNs, North Central London streams/ workshops, training hubs as well as other stakeholders.
The DSN will help support, and provide expert advice to patients with complex diabetes, provide clinical support to GP, Clinical Pharmacist and practice nurses colleagues .

Medicus Health Partners (MHP) is the lead practice for Unity PCN and will be the employing organisation.
The successful applicant will have access to the training, support, and governance systems in place at MHP.

- Demonstrate clinical expertise within the Diabetes specialty and work autonomously, managing own allocated clinics independently.
- To become an essential resource for the multi-disciplinary team and patients and carers with diabetes.
- Carries out diabetes and related diagnostic/therapeutic interventions as an independent practitioner, interprets, writes reports and commences further possible treatment on the basis of the findings.
- Hold joint clinics with allied Health Professionals in order to upskill the wider workforce.
- Produce and present regular online education sessions on all areas of Diabetic care.
- Ensuring the PCN is providing safe practice and under agreed guidelines/protocols to ensure that the highest possible standard is obtained based on competent clinical practice.
- To develop the service in a structured interdisciplinary way.
- To develop interdisciplinary clinical management protocols and work within them.
- To provide, promote and maintain a support and advice service to healthcare professionals managing complex diabetes patients.
