We have a 6 sessions vacancy per week with the potential to increase to 8 for a Diabetes Specialist Nurse with experience of Minor Illnesses to join our thriving, democratic and enthusiastic team at our surgery located in Redditch Town Centre. Redditch is located 15 miles south of Birmingham within easy reach of Stratford, Worcester and surrounding countryside with good transport links and an increasing and diverse population.

Main duties of the job

Undertake a range of nursing assessments and provide appropriate care / treatment in conjunction with GPs according to practice policy and protocols and adhere to the Practice policies, protocols and guidelines at all times.

Strive to improve standards of care and be involved in identifying practice population needs.

Chronic Disease management is an essential part of the role as well as the mentoring of our Health Care Assistants.

Knowledge of the GMS contract

To maintain a safe and effective level of practice and to bring any deficits in training to the attention of the Lead Practice Nurse so that these can be addressed.

Our team consists of 5 GP partners, 4 Salaried GPs, 2 pharmacist, 5 Nurses, a HCA and Phlebotomist supported by a fantastic administrative team. We are passionate about teaching undergraduate students from Birmingham Medical School, FY2 Doctors and GP Trainees.

Main Duties and Responsibilities

  • Assist GP's to help deliver effective implementation of management of pts with chronic diseases in particular Diabetes
  • Support the implementation of evidence-based guidelines in conjunction with the practice nursing team.
  • Encourage the development of integrated services within Primary Care to include integrated practice based teams and multidisciplinary working.
  • Promote your own continuing professional development.
  • Recognize the opportunities for working in partnership with the PCN and Practices involved within the PCN.
  • Participate in an annual appraisal.
  • Support the practice in achieving quality indicators and targets of the GMS contract and any relevant National Service Frameworks.
  • Experience in Chronic Disease Management.
  • Undertake opportunistic screening when clinically required.
  • Perform venipuncture and electrocardiography
  • Undertake wound care/ management/dressing techniques.
  • Leg ulcer management, Doppler and bandaging.
  • Perform cervical screening and promote breast awareness.
  • Administer adult and paediatric injections under an individualised prescription or Patient Group Direction, ensuring safe storage, rotation and disposal of vaccines and drugs.
  • Provide advice on lifestyle issues, assist and support patients in changing their unhealthy behaviours and refer appropriately.
  • Provide an adult and childhood immunisation service working to Patient Group Directions - following appropriate training if required.
  • Deliver travel health care, including vaccinations and advice for patients - following appropriate training if required.
  • Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours and suicidal tendencies and be aware of local, referral procedures.

  • Undertake annual training and demonstrates competency in anaphylaxis and resuscitation techniques Research, Audit and Quality
  • Encourage the development of a practice audit strategy and action plan for Primary Care.
  • Contribute to the provision of equitable, high quality services for patients in the locality.
  • Help support the integration across primary, secondary and tertiary care services.
  • Maintain an interest in national research initiatives and disseminate appropriate information to health professionals if asked to do so.
