Social Prescribing Link Worker at West Enfield Collaborative PCN

Job summary

We are looking to recruit to the post of social prescribing link worker, to work within our primary care network multi-disciplinary healthcare team, providing 1:1 personalised support to people who are referred to them by team members and local agencies. The social prescriber will work across the practices who are part of the West Enfield Collaborative Primary Care Network: Winchmore Hill Practice, Park Lodge Medical Centre, Abernethy House Surgery, The Town Surgery.

Main duties of the job

Wellbeing Co-ordinators will work as an integrated member of an extended team, taking referrals to the Social Prescribing service from three GP surgeries in Enfield. Comprehensive and holistic assessments of need will be completed in order to collaboratively identify support goals and opportunities.

The post holder will support individuals to build independence and personal resilience, enabling self-management of their health and wellbeing. This will be achieved through the creation of Wellbeing plans which will be tailored to individual needs. Key to the role will be the ability to build trusting relationships quickly whilst motivating individuals to make sustained changes, working sensitively and overcoming barriers to access.

Wellbeing Co-ordinators will build and develop relationships with a wide range of services and activities in order to address social and health care needs, sign posting and offering support to individuals to participate and benefit from community based activity.

We are now seeking a compassionate and proactive Social Prescriber / Link Worker to join our expanding multidisciplinary team.The PCN covers three well established GP sites two of which are approved training practices. We believe in working collaboratively and have regular team meetings. Our workforce include GPs, GP and FY2 trainees, Nurses, Pharmacists, Social Prescribers, Care Coordinators, and full complement of administrative staff. You will be employed by the PCN and your time will be divided between the three sites.

Qualifications

Essential

  • Degree and/or equivalent experience in health care/social care or related area.
  • Hold certificate for (or be willing to work towards) Levels1 and 2 Safeguarding - Adults and Children

Desirable

  • Social Prescribing qualification or relevant training
  • Experience working within primary care or the NHS
  • Knowledge of local community assets and support services
  • Experience supporting people with complex or multiple needs

Experience

  • Experience of working with health sector and multiple stakeholders
  • Experience and demonstrable evidence of incorporating patient/client views into the development of services e.g. co-production
  • Good understanding of service improvement, innovation, performance improvement across a range of disciplines
  • Experience of working with statutory sector
  • Experience of holistic interviewing and assessment of individuals to produce an action plan
  • Experience of working with and supporting volunteers
  • An understanding of the VCF sector
  • Ability to use a range of IT programmes
