Job summary
The post holder is responsible
for the delivery of basic practice nursing services care to the practice
population. They will deliver
care within the boundaries of their role, focusing upon supporting patients to
be healthy, monitoring of long-term conditions, health prevention and screening
activities. They will work collaboratively with the general practice team to
meet the needs of patients, supporting the delivery of policy and procedures,
and providing nurse leadership as required.
Main duties of the job
Knowledge of needs of patients with long-term conditions
Aware of accountability of own role and other roles in a nurse
led service
Knowledge of health promotion strategies
Awareness of clinical governance issues in primary care
Knowledge of patient group directions and associated policy
Skills
Clinical skills cervical cytology, immunisation and
vaccination.
Change-management skills and ability to support patients to
change lifestyle
Communication skills, both written and verbal
Ability to communicate difficult messages to patients and
families
Negotiation and conflict management skills IT skills
About us
Windermere Medical Centre is part of the Spinney Group, we are a small and friendly team but very much part of a wider team. As a practice nurse at WMC you will attend regular nurse clinical meetings with the other Nurses within the Spinney Group for support and clinical updates.
Communicate with all members of
the wider health care team; primary, secondary, community and voluntary.
Work as a team to identify and
initiate changes to clinical practice.
Discuss with the practice team any
circumstances that give cause for concern in relation to patient care i.e.
significant events.

Our practice population is around 3300 patients. We are a small friendly team which consists of 3 Gps and a General Practitioner Assistant
You will be expected to work flexibly, carrying out a range of
different tasks and appointments though your main remit will be chronic disease
management cervical smear taking and immunisations. Experience in these areas would be required.
Computer literacy is essential, though we can provide training for our
specific clinical system (EMIS PCS)
We are a small practice and maintain very high standards of patient
care.
It is vital that you are able to work effectively as a part of a small
team, and are committed to making a difference
