Job summary

Cowbridge and Vale Medical Practice are seeking a motivated, proactive Practice Nurse to join our practice team.

We welcome applications from experienced practice nurses and nurses with minor illness/ANP skills.

We are able to offer between 28 and 37.5 hour per week over 4/5 days.

About us

Cowbridge and Vale Medical Practice operates out of a purpose-built Health Centre, located in Cowbridge, in the heart of The Vale of Glamorgan and has plenty of allocated parking. The practice consists of over 8,300 patients with a supportive clinical team of 3 GP partners, 2 salaried GPs, 1 Advanced Nurse Practitioner, 1 GPN, a Nursing Assistant and a Practice Pharmacist, we also have a dynamic administration team.

The practice is an active participant in training programmes for a wide range of clinical staff, we have GP registrars, medical students, GPN trainees, post graduate clinical pharmacists and pharmacy trainees.

Job responsibilities

PRACTICE NURSES JOB DESCRIPTION

The practice nurse is responsible for ensuring that the practice provides an optimum service of nursing care relevant to the practice population.

The main duties and responsibilities are as follows.

1. Meeting patients nursing care needs as identified, or where care has been transferred to her by a GP. This will usually be in surgery but occasional home visits may also be required.

2. Initiating and maintaining health screening and promotion clinics for identified areas of the practice population, recognising where modification of systems or referral is necessary.

3. Managing the nursing care of patients with chronic disease, employing personal recall systems suitable to the practice needs. This includes using the practice computer systems for input of information for recall and screening purposes. Accurate and efficient input of data for GMS contract purposes is also required.

4. The following clinics:

Asthma

Diabetic review

Hypertension Clinic

COPD Clinic

Holiday Vaccination Clinic

Flu Vaccination Clinic

Cervical Smears

Family Planning

Amending or adding to services as needs are identified.

5. Maintaining efficient liaison with all areas of the Primary Care Team and other agencies to assure continuity of care where necessary.

6. Routine treatment in general clinics, including:

Warfarin monitoring

Maintaining Thyroxine register

Wound management

Blood tests

ECGs

and other general Treatment Room skills
