About us

Primary Care Networks (PCNs) form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; to manage financial and estates pressures; to provide a wider range of services to patients and to more easily integrate with the wider health and care system. PCNs were formally ratified in July 2019

West Leeds Primary Care Network (WLPCN Ltd) is a large PCN with a patient list of circa 70,000 patients around the Pudsey / Bramley area. We have a significant workforce in place which brings wonderful opportunities for in-house multi-disciplinary team working. In addition to our Pharmacy Team, we have Occupational Therapists, Social Prescribers, Paramedics, Dietitians, a range of nursing / HCA staff and more which means that the majority of any patient's needs can now be provided on their own doorstep

Primary Care Networks will be required to deliver on seven national service specifications, with which our pharmacists will play a key role:

  • Structured Medication Reviews and Optimisation
  • Enhanced Health in Care Homes Service
  • Anticipatory Care
  • Personalised Care
  • Supporting Early Cancer Diagnosis
  • Cardiovascular Disease Prevention and Diagnosis
  • Tackling Inequalities Locally
