This is an exciting opportunity to join a mature and visionary Primary Care Network that already has a well established and highly skilled team of Primary Care Support Workers (Health Care Assistants).
Our current team of Primary Care Support Workers (PCSW) fulfil a wide range of essential duties critical to the continued operation of our organisation and we are constantly seeking to further develop our services to our practices. This involves upskilling and training our current PCSW team on a regular basis. Due to a member of staff leaving for a new role, we are looking for a new member to join the team. This is diverse and fulfilling work so if you are looking for a new challenge as an experienced HCA this could be the role for you
Main duties of the job
Our current
highly skilled Primary Care Support workers undertake the following tasks:
- Phlebotomy
- Blood pressure checks
- ECG test (but not the
reading and assessing of the results)
- Vaccinations (flu,
B12, Covid) **
- NHS Health Checks
- Physical checks for
patients with serious mental illness (SMI) and learning disabilities (LD)
- Dopplers
- Stocking measurements
- Diabetic foot checks
- Staple / sutures
removal **
- Spirometry
- FeNo (respiratory)
testing **
- Woundcare **
- Smoking cessation
(planned) **
- Long term condition
reviews
- Non clinical dementia
reviews

About us
West Leeds Primary Network provides enhanced clinical services to 6 GP practices in and around Pudsey (Hillfoot Surgery, The Gables Surgery, Mulberry Street Medical Practice, West Leeds Family Practice, Robin Lane Health & Wellbeing Centre and Manor Park Surgery). We have a combined patient list of around 70000 patients and we also have a well developed workforce covering multiple clinical disciplines. This role would be based out of Manor House in Pudsey but the varied nature of the role means that you will also undertake clinics on site at any of our practices as well as performing domiciliary work in our community. You
will be expected to:
- Take a
holistic approach to supporting the assessment of the health and well-being
needs of the individual / patient which includes undertaking basic clinical
observations to accurately assess and monitor a patients blood pressure,
pulse, respiratory rate, temperature, oxygen saturations and take blood samples
- Engage in Collaborative
Care Support Planning resulting in the co-production of the care and support
plan for the individual / patient and implementation of a programme of care
supported by clinical skills protocols and procedures in which training has
been received;
- Co-produce under
supervisions and evaluate personalised care plans against the health and
well-being needs of the individual / patient, their families and cares to take
control of their health and wellbeing.
- Develop trusting
relationships both with the individual / patient as well as members of the PCN
and wider community groups all in pursuit
of improving the health and care of the individual / patient;
- Manage and prioritise
your own caseload under supervision and in accordance with the care needs,
priorities and any urgent support required by individual / patient;
- Utilise the strengths
and capabilities of local communities ensuring that appropriate groups and
organisations are receiving social prescribing referrals from the Primary Care
Support Worker;
- Attend relevant
multidisciplinary team meeting giving information and feedback from current
caseloads, refer patients back to health professionals / agencies and take new
referrals;
- Be responsible for
measuring the impact of patient wellbeing and patient satisfaction through
recognised impact measuring tools and surveys.
