enhanced clinical services for 6 GP surgeries across the Pudsey and Bramley
area, serving a patient population of approximately 70k. Whilst Bramley is
considered to be amongst one of the most deprived areas of Leeds, Pudsey has a
different population profile and does not fall within this category. Public
Health Data has identified that obesity rates amongst the over 55s in our area are
significantly higher than the Leeds average, and we also have an above average
prevalence of diabetes and pre diabetes.

to Population Health Management need to be made. The post holder will be responsible for
delivering and leading highly specialist clinical care to their own caseload
and service within their speciality area. In doing so, they will assess, plan,
deliver and evaluate patient centred programmes of care ensuring that they
