Please note this is an HCA type of role (not a care worker role) so please do not apply if you have no phlebotomy experience as a minimum

This is an exciting opportunity to join a vibrant,fast moving Primary Care Network, covering the Pudsey / Bramley area. The role is a blend of community (domiciliary) work and clinics on site across our 7 practices. No two days are the same and if you have a desire to be upskilled we are very happy to accommodate that. This role goes beyond that of a General Practice HCA; clinical skills will prove to be just as important as relationship building skills (both with patients and staff across our 7 practices). We already have a well developed and established team of HCAs (Primary Care Support Workers) so you will be joining an organisation with great peer support. The workload is also very diverse so if you like variety this is probably the role for you

We will consider part time hours for the right candidate (minimum 30 hours). The salary advertised is the Whole Time Equivalent.

Main duties of the job

You will work as part of a multidisciplinary team making home visits across a population of patients who are isolated, frail, vulnerable or housebound; you will carry out routine screening. You will also have days based in clinic within our surgeries. You will manage and prioritise your own caseload under supervision and in accordance with the care needs, priorities and any urgent support required by individual / patient.

You may also be needed to assist with the ongoing Covid vaccination programme. There will also be other ad hoc tasks you may be asked to fulfil

West Leeds PCN provides services across 7 GP practices in and around the Pudsey / Bramley area with a total patient population of around 70,000. West Leeds PCN already has a significant workforce covering many different clinical disciplines which means that in-house MDT working is part of our work ethic; this benefits our patient population enormously. We have our own Pharmacists, Occupational Therapists, Paramedics, Physiotherapists, Social Prescribers, Dietitians, Advanced Clinical Practitioners, Health & Wellbeing Coaches, Physician Associates and Care Co-ordinators . There is also a team of Primary Care Support Workers already in place so the successful applicant will join a well established and friendly team.

Service Description:

A Primary Care Network (PCN) consists of groups of general practices working together with a range of local providers, including across primary care, community services, social care and the voluntary sector, to offer more personalised, coordinated health and social care to their local populations, typically covering a population of 30-50,000 patients.

It is expected that all areas within England will be covered by a PCN. Over the coming years PCNs will be supported in developing an expanded Primary Care team, with member practices also working alongside other organisations such as Community Trusts and the voluntary sector, in order to help alleviate workload pressures on practices and allow GPs to concentrate on the most complex patients.

NHS England has significant ambitions for PCNs, with the expectation that they will be a key vehicle for delivering many of the commitments in the long-term plan and providing a wider range of services to patients.

These Networks can be structured a number of ways. They should be small enough to still provide the personal care valued by both patients and GP practices, but large enough to have impact and economies of scale through deeper collaboration between practices and others in the local health and social care system.

Job Purpose

The post holder will work within standard operating procedures / pathways to manage their own workload independently but with reference to a supervisor within the clinical area. In doing so, they will support the assessment of patients and implementation of programmes of care ensuring that they practice within their sphere of competence and knowledge. The post holder will work within a range of standard operating procedures and pathways in which they have received training in order to be competent.

Key Responsibilities; you will

- Work as part of a multidisciplinary team making home visits across a population of patients who are isolated, frail, vulnerable or housebound.

- Take a holistic approach to supporting the assessment of the health and well-being needs of the individual / patient which includes undertaking basic clinical observations to accurately assess and monitor a patients blood pressure, pulse, respiratory rate, temperature, oxygen saturations and take blood samples

- Carry out routine screening to identify any risks such as dementia, frailty, falls, diabetic feet and lifestyle factors to signpost or refer appropriately as well as to support assessment.

- Identify the needs and experiences of people with mental health conditions, dementia and learning disabilities in relation to the care they may need.

- Engage in Collaborative Care Support Planning resulting in the co-production of the care and support plan for the individual / patient and implementation of a programme of care supported by clinical skills protocols and procedures in which training has been received;

- Promote the benefits of social prescribing and the work of local groups and communities including responsibilities to educate other members of the PCN team on what services and support is available within the wider community.

- Co-produce under supervisions and evaluate personalised care plans against the health and well-being needs of the individual / patient, their families and cares to take control of their health and wellbeing.

- Develop trusting relationships both with the individual / patient as well as members of the PCN and wider community groups all in pursuit of improving the health and care of the individual / patient;

- Manage and prioritise your own caseload under supervision and in accordance with the care needs, priorities and any urgent support required by individual / patient;

- Utilise the strengths and capabilities of local communities ensuring that appropriate groups and organisations are receiving social prescribing referrals from the Primary Care Support Worker;

- Attend relevant multidisciplinary team meeting giving information and feedback from current caseloads, refer patients back to health professionals / agencies and take new referrals;

- Be responsible for measuring the impact of patient wellbeing and patient satisfaction through recognised impact measuring tools and surveys.

Skills and Experience; you are

- Empathetic and caring, individual / patient focussed;

- Working in healthcare setting and delivery of care for patients, carers and their families;

- Competent to carry out clinical observations, screening and assessments under supervision;

- Hard working and motivated to achieve good outcomes for individuals and patients;

- Able to work effectively as part of a multidisciplinary team and to build effective relationships within that team;

- In possession of excellent verbal and written communication skills;

- In possession of good IT skills and able to use Office packages;

- Organised with effective time management skills;

- Able to solve problems with analytical ability;

- Able to work with confidential information and handle, store and maintain confidentiality at all times;

- Car owner with valid UK driving licence.

Qualifications

- Educated to at least GCSE level or equivalent

- Health and well-being qualification is desirable as is NVQ level 3 in Health and Social Care or other relevant professional academic qualification;

- Must be willing to participate in any clinical or other skills training identified as part of this role;
