FRAILTY / DEMENTIA PATIENT COHORTS
This is a very exciting opportunity to take on a brand new role within our PCN and make it your own. Our PCN has a pioneering Frailty Team that does extraordinary work and we need a Nursing Associate to boost capacity within this very busy team. This will be a dual role dividing time between frail patients and those presenting or diagnosed with dementia. If you love to be able to provide holistic care for your patients without those traditional obstacles such as working in silos, not having instant access to other clinical teams etc then this is most definitely the role for you. West Leeds PCN has a total workforce of around 50 people in multiple clinical disciplines so the opportunities to tap into another colleagues' expertise for advice and guidance is almost limitless. Indeed, if you are not out in the community you will be at Manor House (PCN Headquarters) where you would have instant access to the rest of the workforce. This kind of seamless collaborative working has become the norm for us and the benefits to our patient population are significant. It also produces a job satisfaction that is hard to replicate elsewhere.
We are looking for someone to work up to 37.5 hours a week but part time (minimum 30 hours) would be considered for the right candidate
Main duties of the job
Work
with the Frailty team to ensure all frail patients have a full and holistic health
review
Help
to prevent those with moderate frailty
tipping into the severely frail category with proactive and responsive care
Work
with dementia sufferers (who are not necessarily frail) and their families to
review patients with a diagnosis. Provide
the best care for this cohort in conjunction with St Marys and other mental health
organisations
Review
patients with dementia type symptoms with a view to referral to St Marys for formal
diagnosis
Will
take a lead on dementia MDT meetings as PCN dementia champion
Work with the healthcare
assistants within the Frailty Team, ensuring all tests and observations for the
first part of the frailty review are done prior to the Advanced Practitioners
completing the review
Treat all patients and their
families with compassion, kindness and dignity
Be equally comfortable
working alone and as part of a team
Be
an excellent communicator
Provide
proactive care whilst improving your patients (and loved ones )quality of life
Liaise
closely with other teams / organisations locally and citywide to boost
healthcare for your patients
Take
an interest in continued professional development, upskilling at every
opportunity
Provide
cover if needed for other clinical PCN teams

developed multi skill Frailty team and we are hoping to recruit a Nursing
Associate to join them. The workload is varied and interesting plus
you will benefit from mentorship as well as professional development.

health care, mainly to our older frail population. The team performs comprehensive health reviews,
some acute visits, Advanced Care Planning and some dementia reviews. The successful recruit will contribute to the
whole of this teams valuable work, improving the health and wellbeing of this
vulnerable patient cohort. You would also
have instant access to the advice and guidance of the rest of the PCN clinical
/ practitioner teams, making for true multi-disciplinary team working in-house.
In addition to this,

and mentorship in your daily worklife.
