Groves Health is looking for an experienced and motivated part time Practice Nurse to deliver high quality care to patients in our four general practice sites across South West London and Surrey.
Working hours will be by arrangement and can be flexible.
Main duties of the job
Key responsibilities:
Conduct thorough assessments of patients' health conditions.
Formulate and implement care plans
Provide treatment as necessary
Perform treatment room duties and CDM clinics
Collaborate with other healthcare professionals to ensure optimal patient care.
Maintain accurate and up-to-date patient records
Offer health education and counselling to patients and their families
Stay current with the latest medical advancements and best practices
Qualifications
Current and valid registration with the Nursing and Midwifery Council (NMC)
Proven experience as an Practice Nurse
Child and adult immunisations, Cervical screening, Chronic Disease management, would care. Spirometry an advantage
Strong communication and interpersonal skills
Ability to work independently and as part of a multidisciplinary team
Commitment to providing high-quality patient care

Job Summary
- To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population.
- The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, adult and child vaccination clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.
Primary Responsibilities
- Identify, manage and support patients at risk of developing long-term conditions
- Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
- Provide vaccination and travel medicine services
- Provide wound care (ulcer/ Doppler etc.) to patients
- Request pathology services as necessary
- Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary specialist care as required
- Maintain accurate clinical records in conjunction with extant legislation
- Ensure snomed codes are used effectively
- Maintain chronic disease registers
- Maintain vaccination programmes for adults and children
- Provide cervical cytology
- Priorities health issues and intervene appropriately
- Recognise, assess and refer patients presenting with mental health needs
- Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)

- Act as the audit lead, effectively utilising the audit cycle
