Job summary

Park Medical Centre would like to appoint an Advanced Clinical Practitioners (Advanced Nurse Practitioner/Advanced Paramedic Practitioner)- we are looking for 25.5 hours working on Tuesday, Wednesday and Thursday (10am to 6.30pm).

Main duties of the job

Your role would be to work collaboratively with the General Practice team, to meet the needs of patients supporting the delivery of policies and procedures and providing leadership as required. They will be required to assess, diagnose, plan, implement and evaluate treatment. They will be required to clinically examine and assess patient needs and demonstrate safe decision making and expert care. The candidate will need to show the ability to work independently, as well as working within a diverse team to provide high quality care to the patient population registered at Park Medical Centre. Main duties include assessing acute illness presentations, prescribing, developing treatment plans, ongoing Long Term Condition Management, facilitation of Specialist referrals and Palliative/End of Life Care. Good IT skills are required within this role to ensure accuracy of data on to patient record in EMIS.

You will be working alongside a team of 3 Partners (2 x GP 1 X ANP), 6 Salaried GPs, Registrar(s), 3 Nurse Practitioners (2.3 FTE), 2 Practice Nurses, 3 HCAs, Clinical Pharmacist, Pharmacy Technician and our virtual ward Nurse. In addition we are supported by our PCN colleagues including: PCN Clinical Pharmacist, Pharmacy Technician, Occupational Therapist, Social Prescriber, Mental Health Practitioners and First Contact Physios.

Job responsibilities

Triage urgent care requests, consulting with patients or directing them appropriately.

Clinically assess the patient and diagnose and treat as appropriate, both in the surgery and in the patients home environment.

Provide accurate information regarding patient requirements when the patient needs to be directed to another clinician.

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.

Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.

Work with patients in order to support compliance with and adherence to prescribed treatments.

Provide information and advice on prescribed or over-the-counter medication, on medication regimens, side-effects and interactions.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.

Support and manage health needs of women presenting for family planning or sexual health consultation.

Assess, identify and refer patients presenting with mental health needs.

Participate in vaccination and immunisation programmes, for both adults and children, if required.

To deliver a high standard of patient care as Advanced Clinical Practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.

Continual Professional Development in line with Clinical and Professional requirements. Also provide support in the professional development of the Nursing team.
