Scope and Purpose of the Role

The post holder will deliver a consistently high standard of patient-centred care across the Community setting, Same Day Hub, and Urgent Treatment Centre (UTC), utilising advanced autonomous clinical skills supported by a broad and in-depth theoretical knowledge base.

The role requires confident, independent practice without direct supervision and the ability to manage an autonomous clinical caseload. The post holder will assess, diagnose, and manage patients presenting with undifferentiated and undiagnosed conditions, ensuring safe, effective, and timely care.

Job Dimensions

• Independently manage and prioritise a complex clinical workload, responding effectively to patient need while promoting patient choice and improving access to services.

• Develop, utilise, and maintain effective referral pathways to local acute NHS Trust services, including Diagnostic and Treatment Centres.

• Provide clinical leadership, mentorship, and support to colleagues and junior clinicians, contributing to the development and maintenance of high clinical standards.

Primary Duties and Areas of Responsibility

A. Clinical Responsibilities

• Practice with a high level of professional autonomy and accept accountability for clinical decision-making.

• Act as the first point of clinical contact for patients presenting with undifferentiated, undiagnosed, or complex conditions.

• Use advanced clinical skills in history taking, physical examination, clinical reasoning, and decision-making to formulate differential diagnoses and management plans.

• Initiate, request, and interpret appropriate invasive and non-invasive diagnostic investigations, including blood tests and imaging.

• Prescribe safe, effective, and cost-efficient medication in accordance with current legislation and best practice (where an independent prescribing qualification is held).

• Deliver evidence-based, individualised, and cost-effective care in line with national and local clinical guidelines.

• Provide holistic care to patients and their families, developing ongoing management and support plans with a strong emphasis on prevention, self-management, and shared decision-making.

• Promote health and wellbeing through patient education, motivational interviewing, screening, and health promotion initiatives.

• Refer patients directly to appropriate internal and external services and agencies as required.

• Contribute to the achievement of quality, safety, and performance indicators to maintain high standards of patient care and service delivery.

B. Teaching and Mentoring Responsibilities

• Foster and sustain a positive learning environment for patients, colleagues, and learners.

• Contribute to the planning, delivery, and evaluation of education and training for a range of staff groups, including:

  • Medical students

  • Nurse Practitioner students

  • GP Registrars

  • Health Care Assistants

C. Professional Responsibilities

• Promote and embed evidence-based practice, contributing to audit, service evaluation, and practice-based research where appropriate.

• Continuously monitor and evaluate personal clinical practice through audit, peer review, reflection, and other quality assurance processes.

• Maintain professional registration and meet all requirements for continuing professional development.

• Practise in accordance with the most recent NMC Code of Professional Conduct (or relevant professional governing body).

• Ensure accurate, timely, and comprehensive documentation of all patient consultations in line with professional, organisational, and legal standards.

• Remain informed of relevant national and local health policies and work collaboratively with the multidisciplinary team to support implementation of service developments.

• Work effectively and collaboratively with colleagues across the practice and wider healthcare system to deliver integrated, high-quality care.

Person Specification

Qualifications

Essential

• Experience working in either Accident and Emergency / Minor Injuries or GP practice.
• Nurse practitioner, Emergency care degree, relevant degree or equivalent level training in related field. Experience in assessment, diagnosis and management of patients.

Desirable

• Emergency Nurse practitioner course. Accident and Emergency course or Enhanced Clinical Examination or Consultation Module. Minor injuries and Minor Illness course. Independent Nurse Prescribing. Primary Care experience. Mentorship/and or Leadership.

Experience

Essential

• Experience of working either in an Accident and Emergency, Minor Injuries Unit or General Practice.
• Taking history from a patient.
• Has an understanding of local and national issues relating to urgent care.
• Up to date knowledge of DH and NICE guidance.

Desirable

• Some knowledge in the management of minor illness/injury.
• Fracture management/manipulation and simple plastering.
• Suturing and wound closure (maintaining aspetic technique).

• Understanding of chronic disease presentations for primary care including heart disease, diabetes, hypertension, asthma, chronic lung disease.
• Understanding of vaccinations and immunisations for children and adults.
• IT skills to perform library searches enabling enhancement of theoretical knowledge.
