Main duties of the job

As an Advanced Nurse Practitioner at Priory Medical Centre, youll play a pivotal role in delivering high-quality, patient-focused care across both planned and same-day services. Your day-to-day work will be varied, autonomous, and rewarding.

Youll take the lead in managing Long Term Conditions, with a specialist focus on Diabetes, providing comprehensive reviews, personalised care plans, medication optimisation, and proactive health coaching. Your expertise will help patients take control of their health and improve outcomes across our population.

Alongside this, youll run same-day acute triage clinics, assessing, diagnosing, and treating a broad spectrum of undifferentiated conditions. Youll use your advanced clinical skills to make safe, effective decisions, request and interpret investigations, prescribe independently, and escalate or refer when appropriate.

Youll work closely with our GPs, nurses, pharmacists, paramedics, and wider multidisciplinary team, contributing to a supportive, progressive environment. Youll also be involved in continuity of care, safeguarding, quality improvement, and helping shape service delivery as primary care evolves.

About us

Priory Medical Centre is a well-established Warwick GP Partnership with a list size of approximately 18,900 patients and one branch surgery. We have 5 GP Partners and a full complement of additional staff including Salaried GPs, Advanced Nurse Practitioners, Practice Nurses, Health Care Assistants and administrative support. We are a recognised training practice for GP Registrars.

In June 2021, the practice moved into a brand new purpose built surgery designed with patient care in mind. The new building will be a modern, energy efficient & fit for purpose building that will enable the practice to provide better services to our patients. As a result of having the new facilities available, the practice closed the main Cape Road surgery, along with the Cape Road branch surgery. The new facilities will act as the main surgery, with a branch surgery still available to patients on Woodloes Park at the following address: 1 Brese Avenue, Woodloes Park, Warwick, CV34 5TS.

Job description

Job responsibilities

The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Develop, implement and embed health promotion and wellbeing programmes

b. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

c. Implement and evaluate individual specialised treatment plans for chronic disease patients

d. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

e. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

f. Support wound care (ulcer/Doppler etc.) by providing advice to the nursing team

g. Provide specialist clinics such as contraception initiation etc.

h. Request pathology services as necessary

i. Process and interpret pathology and other test results as required

j. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

k. Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectively

l. Maintain chronic disease registers

m. Develop, implement and embed an effective call/recall system

n. Be responsible for the implementation of an effective immunisation programme

o. Chaperone patients where necessary

p. Prioritise health issues and intervene appropriately

q. Support the team in dealing with clinical emergencies

r. Recognise, assess and refer patients presenting with mental health needs

s. Support vaccination programmes for adults and children

t. Be an Independent Nurse Prescriber, adhering to extant guidance

u. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

v. Contribute to practice targets (QOF etc.), complying with local and regional guidance

w. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

x. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual

y. Support the clinical team with all safeguarding matters in accordance with local and national policies

z. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

aa. Deliver opportunistic health promotion where appropriate

b. Act as the IPC lead for the practice, ensuring compliance, undertaking audit as necessary

c. Monitor and ensure the safe storage, rotation and disposal of medicaments

d. Undertake overall management of the nursing team, providing guidance when necessary, acting as a mentor to students and newly qualified staff

e. Participate in local initiatives to enhance service delivery and patient care

f. Support and participate in shared learning within the organisation

g. Develop an area of specialist interest, taking the lead within the organisation

h. Continually review clinical practices, responding to national policies and initiatives where appropriate

i. Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)

j. Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required

Essential

  • Registered Nurse with Nursing and Midwifery Council
  • Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
  • Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
  • Qualified Independent Nurse Prescriber on the NMC register
  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
  • Meets the standards for registered ANP working at advanced level
  • Minor illness qualification
