Job summary
Working in accordance with the NMC Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients. To include management of long term conditions, wound care management, patient health checks, health screening, travel health, cervical screening and Adult and Child immunisations.
The post-holder will work with reference to Practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.
The post-holder will have the required level of training and competence and will work within patient group directions where these are available. A nurse with prescribing qualifications will use them appropriately.
Main duties of the job
Management of Long-Term Conditions Diagnosis, monitoring and
development of individual management plans.
Therapeutic Monitoring Checking compliance with
and adherence to appropriate treatments using a holistic patient-centred
approach. Recognising abnormalities. Identifying the impact of treatment and
implementing or recommending changes as appropriate.
Wound Care Management. Assessing routine wounds
including trauma. Assessing and prescribing to support good wound management
in complex situations.
Patient Health Checks Identifying significant abnormalities.
Obtaining appropriate information using a lifestyle questioning approach and
identifying appropriate health promotion issues.
Risk Assessment Recognising issues and
gathering sufficient information to refer (e.g. drugs; domestic violence; child
protection; vulnerable adults; senior patients; social problems)
Health Screening Recognising issues and
gathering sufficient information to refer (e.g. womens and mens health,
sexual health, older people). Undertaking monitoring tasks (e.g. smears) and
providing advice as appropriate. Providing in depth monitoring and advice as
appropriate and working with the
patient in deciding on management plans.
Travel Health Administering injections
and providing guidance in accordance with guideline. Provision of specialist
and evidence-based support to the team.
Immunisation (Adult and Child) Administering appropriate
immunisation autonomously.
About us
Wordsley Green Surgery is a practice with 5 GP Partners and 5 Salaried GPs working with a multi-disciplinary team. There is excellent nursing and administrative support. We are high QOF and quality target achievers. We offer a wide range of services, including advanced minor surgery and ICB commissioned community clinics for Gynaecology and Vasectomy, supported by the nursing team. The systems currently used in the practice are EMIS Web, Docman and Accurx for online consultations.

- Assist in and perform routine tasks related to patient care.
- Cervical Screening
- Wound care management and removal of Sutures/Clips
- Management of Long Term Conditions
- New Patient Medicals/Urinalysis
- Travel Advice
- ECGs
- Hypertension Management including 24hr BP Monitoring
- Routine immunisations for both adults and children, for example: child immunisations, travel vaccines, influenza, pneumococcal, shingles, RSV etc. (list not exhaustive).
- Assist with all minor surgery procedures, gynae and vasectomy clinic when required
- Requesting pathology tests, for example urine culture, swabs
- Following agreed clinical protocols with referral to GPs as appropriate
- Be responsible for the monthly drug checks, including the emergency drugs
- Give injections as prescribed by the Doctors, for example: prostap, zoladex, testosterone, depo-provera, vitamin B12 etc. (list not exhaustive).
- Pill checks
Other tasks will include
- Maintaining and cleaning equipment used by the nurses and GPs
- Risk Assessments
- Audits
- Maintaining GP and Nurses rooms, stocking and rotating items as required
- Chaperoning and assisting patients who are being examined by another clinician.
- Maintain general tidiness and cleanliness of nurses and treatment rooms
- Participation in administrative systems in the Practice
- At all times there will be a need to maintain accurate records
- A duty to advise the Practice Manager or partners in Practice Managers absence of potential problems or errors within the range of assigned tasks
- Attend and participate in any Practice meetings when required.
