"One Ellesmere Port Primary Care Network (PCN) is a
collaboration of the 6 GP Practices from across the town. Working
together at scale to develop better services for our patient population of
approximately 72,000 patients."
"Whitby Health Partnership is a core member Practice of
the PCN and the second largest Practice."
"Whilst the successful candidate will be employed by,
and work at Whitby Health Partnership focusing on their patient population,
they will join a valued and existing team of 5 Mental Health Occupational
Therapists who work across the PCN. This team meets regularly which allows peer
support, and best practice sharing."
Main duties of the job
To provide a lead in supporting the PCN in triaging mental health concerns raised by on the day patient requests, providing specialist advice and support within our GP surgery.
To review patients presenting with mental health issues, consider a range of support and treatment interventions, providing them with highly specialised advice and signposting when appropriate and liaising with GPs and MH teams
To offer brief interventions as needed e.g. problem solving, decision making, building resilience, building on personal strengths, risk de-escalation, medication advice and prescribing if a registered NMP
To provide a navigation role to help GP's make appropriate referrals to the relevant community and secondary care services - bridging the gap between primary care services such as IAPT and secondary care, working in partnership across the system.
