Job summary

We are seeking a Digital and Transformation Lead to play a pivotal role in driving forward the digital agenda and transformation initiatives within the Carlisle Rural Primary Care Network (PCN). If you have the skills and experience we are looking for we can offer a competitive range of benefits commensurate with AfC Band 7.

Main duties of the job

The post holder will work with clinical and operational teams within the Primary Care Network (PCN) based at Brampton Medical Practice to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, resource and tools to deliver changes in a collaborative way. The post holder will plan, coordinate and directly support delivery of transformation activity with a view to improving patient access, staff and patient satisfaction, and the efficiency and sustainability of services. The post holder will work to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered.

About us

We are a popular and forward-thinking training practice of over 16,500 patients operating from three sites in the beautiful Cumbrian countryside of the Northern Pennines and Eden Valley, close to the great border city of Carlisle, within easy reach of the Lake District, the Borders and Northumberland.

We are part of Carlisle Rural PCN and have an excellent team of Healthcare Professionals and supportive administration teams. We are also a dispensing practice with dedicated Medicines Management Team and Dispensing Team.

The Practice is committed to safeguarding adults and children therefore if the post is subject to the Rehabilitation of Offenders Act (ROA) 1974 (Exceptions) Order 1975 (as amended) the successful candidate(s) will be subject to a criminal records check with the Disclosure and Barring Service (DBS). The cost of the DBS must be met by the successful candidate(s) through salary deduction. DBS charges are as follows if applicable to the post:

Standard Check: £43.50. Deducted at £14.50 from 1st month salary and over the following 2 months

Enhanced Check: £61.50. Deducted at £20.50 from 1st month salary and over the following 2 months

Main duties of the job

As the Digital and Transformation Lead, you will:

Develop and implement digital strategies aligned with practice objectives and NHS/ICB priorities.

Lead transformation projects from conception to successful delivery, ensuring they enhance patient care and operational efficiency.

Oversee data management and analytics to inform decision-making and ensure compliance with GDPR.

Implement and optimise digital technologies, providing training and support for staff across all areas of the practice

Foster collaboration among healthcare providers and external partners, representing the PCN and practices in digital health forums

Advise on innovative opportunities and support the PCN to maximise benefits from innovation, through the sharing of learning and best practice

Develop and implement mechanisms for assessing the impact of agreed changes

Provide feedback to PCN leadership including the practice partners, Business Manager and Practice Managers on progress of current initiatives, capacity for change, progress and issues so that plans can be adapted in response.

Lead and manage the clinical data administration function ensuring accuracy and consistency of patient data as well as achievement and maximisation of practice targets

Primary responsibilities

Develop and implement a comprehensive digital strategy aligned with the PCN and Practice objectives and NHS digital priorities.

Identify opportunities for digital innovation and transformation to improve service delivery and patient outcomes.

Project Management:

Lead digital transformation projects from inception through to successful delivery, ensuring projects are completed on time and within budget

Coordinate with internal stakeholders and external partners to achieve project goals and outcomes.

Data Management and Analytics:

Oversee the management and utilisation of healthcare data to drive insights and inform decision-making processes.

Ensure compliance with data protection regulations and standards (e.g., GDPR) in all digital initiatives.

Lead and manage the clinical data administration function ensuring accuracy and consistency of patient data as well as achievement and maximisation of practice targets.

Technology Implementation:

Evaluate, recommend, and implement digital technologies and systems that support clinical and administrative functions

Provide training and support to staff to optimise the use of digital tools and system

Change Management:

Lead change management efforts related to digital transformation, promoting a culture of innovation and continuous improvement

Engage and collaborate with healthcare professionals to facilitate adoption of new digital solutions and workflows

Partnership and Stakeholder Engagement:

Build and maintain effective partnerships with external stakeholders, including NHS organisations, local authorities, and third-party vendors.

Represent the PCN in digital health forums and working groups to share best practices and contribute to policy development.

Quality Improvement:

Monitor and evaluate the impact of digital initiatives on patient care quality, clinical outcomes, and operational efficiency

Implement quality improvement measures based on data-driven insights and feedback from stakeholders

Regular Review of templates, systems and coding within or other practice systems to ensure effective standards coding and patient recalls are being adhered too
