Job summary

We are looking for a new member to join our team due to an increasing practice population. We are flexible with working hours and would consider either, full/part time hours or compressed hours. We have weekly clinical meetings in addition to daily GP support when required. We also offer excellent management and administrative support. Additional benefits include, mutually negotiated working hours, enhanced contractual terms, an annual performance bonus, FREE snacks throughout the week and an additional half day annual leave in December! We currently have approximately 6100 patients, however our list size continues to increase. We use Emis Web, clinical system. We are a training practice, taking nursing students, as well as, training GPSTs and Foundation Doctors. We also train Medical Students from Manchester. As a training practice, we encourage staff development for all our staff. We are also a research practice, collaborating with the North West Regional Research Delivery Network. We are a high achieving QoF practice and enjoy supportive working with our Primary Care Network. The successful candidate will work alongside our existing team providing excellent patient centred care.

Main duties of the job

We are looking to recruit a Practice Nurse, ideally, with experience of long term condition management i.e. Diabetes, Asthma/COPD, CHD and Hypertension. The successful candidate should also have experience of cervical cytology, and spirometry, although additional training will be provided if required. Immunisation and vaccination experience, in both childhood immunisations and travel would also be advantageous. Experience in any of these areas is preferred, however, as above, if necessary, further training would be provided.

About us

Clinically, our practice currently consists of 2 GP partners, a salaried GP and two sessional GPs. Our clinical team is also complimented by a Physician Assistant, an Advanced Level Practice Nurse, two Nurse Associates and a trainee Health Care Assistant. Our team also includes, two Clinical Pharmacists and a Pharmacy Technician. Our practice also has a well established reception, admin and management team, who help with the day to day running of the practice. We value ourselves as being a welcoming, flexible, innovative practice and are keen to look at proactive, new and improved ways of working to help improve the care of our patients.

Job responsibilities

To provide a comprehensive nursing service to the practice population to meet health care needs.

To work as part of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population. The post holder will be aware of a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health prevention, well women and well man clinics.

DUTIES AND RESPONSIBILITES

To develop, implement and embed health promotion and well-being programmes

To implement and evaluate individual treatment plans for chronic disease patients that promote health and well-being

To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health

To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE.

To diagnose and manage acute and chronic conditions

To prescribe medication for patients in accordance with PGDs and own scope of practice

To review patient medications to enhance compliance

To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages

To oversee the provision of minor wound care services to patients

To co-ordinate the provision of travel medicine services

To request pathology services as necessary

To process pathology results as required

To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

To maintain accurate clinical records in conjunction with extant legislation

To ensure read codes are used effectively by all members of the nursing team

To ensure the appropriate maintenance and use of chronic disease registers

To develop, implement, embed and participate in well woman clinics

To develop, implement, embed and participate in well man clinics

To chaperone patients where necessary

To assist GPs with minor surgery when required
