Job summary
We are looking to recruit a
clinical pharmacist to develop medicines optimisation services within our Whitmore
Reans Health Centre.
The successful candidate will
have a patient facing role in managing long-term conditions; working directly
with patients to assess and treat conditions, as well as promote self-care.
The candidate will have
excellent written and verbal communication skills, experience or influencing
others and conducting clinical medication reviews.
The successful candidate will
work as part of a multidisciplinary team to develop and run processes for
repeat prescription re-authorisation, management of medicines on transfer of
care and systems for safer prescribing. You will also undertake structured
medication reviews for patients in care homes and community.
In return we will support to
develop the role of clinical pharmacist through a training pathway (currently
the CPPE pathway) with support of General Practitioner clinical supervisor and
Senior Clinical Pharmacist.
Experience in General Practice
would be advantage but is not essential if the application is clinically up to
date and understands the process of a full medication review in the primary
care setting.
Knowledge of EMIS and Docman 10
would be advantageous, but if not, training will be provided.

About us
The main surgery is located off Lowe Street in Whitmore Reans with close proximity to the Avion Shopping Centre. It is within easy reach by public transport and it is close to the main City Centre.
The branch surgery is in Pendeford near the Morrisons Supermarket off Whitburn Close. Both surgeries are in the Health Centres and are equipped to serve patients in a primary care environment.
The second branch surgery is in Ednam Road near to the Aldi Supermarket and is within easy reach by public transport.
