Job summary

Rotton Park Medical Centre, situated in Edgbaston, is looking to employ a Practice Nurse to join it's established medical team for 2 days a week ( 10 hours). Applciations from both established and newly qualfied Practice Nurses are welcome. The successful candidate will work alongside and be supported by the existing Practice Nurse and HCA. There are also excellent support from 2 GP Partners and 2 Salaried GPs and an established team of administrative staff. The successful candidate will be required to do the normal Practice Nursing duties as outlined in the job description and specification. Informal queries are welcome.

Main duties of the job

The Practice Nurse will be expected to independently run clinics to provide routine nursing care to our patients. This includes cervical smears, immunisations, travel advice, wound care, phlebotomy, B12 and depot contraceptive infections, suture removal, chronic disease reviews and care plans ( including diabetes , asthma, COPD, hypertension), NHS health checks. The Practice Nurse will be responsible forordering and managing stock and conducting regular audits ( e.g. infection control and waste management, emergency drugs and equipment, PPE). They will also ensure compliance with CQC regulations.

About us

Rotton Park Medical Centre is situated in Edgbaston in a purpose built building with secure off road parking. We have just over 5000 patients. We have a small friendly team. The clinical team here is well established with 2 GP Partners, 2 salaried GPs, 1 Practice Nurse and 1 HCA. We also benefit from the support of an experience administrative team.

Rotton Park Medical Centre is rated Outstanding by CQC. We also achieve high QOF points with low exception coding rates.

We are very keen to trainee and support and develop our team. We have been a training practice for many years for both Foundation Drs and GP trainees. Rotton Park Medical Centre has also support the training of 2 Nurses to complete the 12m Fundamentals of Nursing course. And there is a high level of support for the nursing team with all members of the team being included in the delivery of QOF and chronic disease management.

Job Title:Practice Nurse

Reporting to:The Practice Manager/Partners

Role summary: The post holder is responsible for the delivery of basic practice nursing services care to the practice population. Supported by senior nurses within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.The following list serves to illustrate the scope and responsibilities of the post and is not intended to be an exhaustive list of duties. You will be expected to perform different tasks as necessitated by the development of this role as the practice develops, and the overall business objectives of the practice change

Clinical practice:

Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being

Implement and evaluate individual treatment plans for patients with a known long-term condition.Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care

Deliver opportunistic health promotion using opportunities such as new-patient medicals. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions

Support patients to adopt health promotion strategies that promote patients to live healthily, and encourage principles of self-care

Assess and care for patients presenting with uncomplicated wounds

Support and advise women requesting information relating to family planning needs

Support and manage health needs of women presenting for cervical cytology consultations

Recognise, assess and refer patients presenting with mental health needs in accordance with the National Service Framework (NSF) for Mental Health

Implement and participate in vaccination and immunisation programmes for both adults and children

Advise, support and, where appropriate, administer vaccinations for patients travelling abroad

Promote and deliver evidence-based care for patients presenting with aural conditions

Assist senior practitioners in providing minor-surgery sessions

Be responsible for the call and recall of patients requiring care e.g. cervical smears, childhood immunizations, chronic disease reviews

Do ECGs. Give immunisations in line with PGDs and PSDs. Do Cervical Smears, chronic disease review and produce care plans, wound care, suture removal, NHS Health checks, smoking cessation advice, IM infections including b12 and depot contraceptive injections, phlebotomy. Review and implement requirements for QOF and LISs and DESs.

Communication:

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

Communicate with and support patients who are receiving bad news

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating

Utilise communication skills to support patients to adhere to prescribed treatment regimens

Anticipate barriers to communication and take action to improve communication

Estimate and maintain effective communication with individuals and groups within the practice environment and external stakeholders.

Act as an advocate when representing the patients and colleagues viewpoints to others.

Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC)

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
