An exciting opportunity has arisen within the practice for a practice nurse to join our friendly and supportive team. We are a forward thinking practice who have a patients centered care approach, serving 10500 patients. Candidates should be willing to work in an involving role be enthusiastic, empathetic and have a flexible approach. Excellent communication skills are essential to this role.
Main duties of the job
Knowledge of needs of patients with long-term conditions
Cervical cytology, immunisation and vaccination skills
Spirometry monitoring
Change management skills and ability to support patients to change lifestyle
Excellent communication skills, both written and verbal
Ability to communicate difficult messages to patients and families
Aware of accountability of own role and other roles in a nurse led service
Knowledge of health promotion strategies
Awareness of clinical governance issues in primary care
Knowledge of patient group direction
Good IT skills
Excellent record keeping
Experience of TPP S1
Membership of a professional body
Motivated and enthusiastic

Kingston Health is a friendly practice of 10500 patients based over 2 sites covering both west and east of the city.
We have 4 GP partners, 1 Physician Associate. 1 Pharmacist , 2 Practice Nurses, 2 Health Care Assistants supported by ourknowledgeable administrative team. We are also a training Practice with F2s.

Clinical
To
manage patients with chronic disease, running specific clinic sessions and
maintaining robust call and recall systems
Participate
in national screening work such as cervical cytology and deliver the national
immunisation and vaccination schedule
Provide
travel health advice and administer appropriate vaccinations
Provide
directly and assist GPs with contraceptive services
Interact
with patients/parents/carers and provide appropriate ongoing support to meet
identified needs
Undertake
health promotion to help educate patients
Select,
carry out and interpret results of specific tests if required
To
maintain accurate patient records and enter onto the computer using Ardens templates
Ensure
accurate completion of all necessary documentation associated with patient
health care
To
collect data and participate in regular Practice audit and research projects
To
contribute to the preparation of any Practice development plans
To
maintain own annual registration with the NMC and adhere to Code of
Professional Conduct
To
maintain a safe and therapeutic environment for all nursing team staff,
patients, carers and co-workers from other agencies. Ensure all statutory
regulations (e.g. Health & Safety, Infection Control etc) are implemented and
adhered to
To
promote a professional approach at all times by attire and attitude to
patients and staff both in and out of the Practice
To
maintain own continuous professional development and ensure PDP kept
up-to-date in line with the requirements of the Practice
Attend
