Job summary
Ash Trees Surgery is a 29,000 patient general practice on the Lancashire, Cumbria border. We are seeking a motivated and passionate Urgent Care Practitioner to join our friendly and supportive Practice team.
We are a training practice so support and further training will be available.
The role is to work within our Urgent care team, providing excellent care for our patients who require urgent treatment. The practitioner would be able to take a full history from the patient, assess, diagnose, and treat as appropriate including referral onto other practitioner's or secondary care if required.
Main duties of the job
Working within the urgent care team you will be able to diagnose and treat patients of all ages presenting with acute problems and exacerbations of long term conditions, via telephone and face to face consultations.
You will need to prioritise the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.
You may be required to do home visits for our housebound patients and support our visiting team.
Being an independent prescriber is a useful skill however not mandatory to the role.

Ash Trees Surgery covers the semi-rural area of the North Lancashire and South Cumbria border across 6 sites.
We work in a beautiful part of the country with the Lake District and Yorkshire Dales close by with good travel links via the M6 and West Coast rail line.
Our clinical team includes 20 GPs, 1 Advanced Nurse Practitioner, 2 Nurse Practitioners, 2 clinical pharmacists, 1 paramedic, 8 practice nurses, 1 NA, 2 HCAs plus support from the allied health professionals who are part of the PCN team.
We also have administrative support staff and patient advisors who all help with the running of the surgery and provide support to clinical staff as well as patients.

As an Urgent Care Practitioner, demonstrate advanced clinical competence and a knowledge base beyond those associated with conventional nursing/ health professional roles.
Undertake consultations with patients as an autonomous practitioner and using own clinical judgement to diagnose, treat or refer patients
Diagnose and manage both acute and chronic conditions, integrating both drug and non drug based treatment methods into a management plan.
Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols.
Ensure patients receive high quality clinical care, delivered in a timely manner
Ensure complete and accurate documentation of each and every patient contact
Prescribe and review medication in accordance with practice and national protocols
Work as an autonomous practitioner and as part of multi-disciplinary and multiagency teams in order to ensure patient needs are met
Adhere to NMC Code of Professional Conduct (or equivalent professional body) and at all times work within the scope of professional practice.
Maintain confidentiality with regard to information pertaining to patients and staff
Deliver care according to NICE guidelines and evidence-based care
Act as a role model in the observance of equality and diversity good practice
