An exciting opportunity is available for a motivated Nurse Associate to join our dedicated and progressive team. The succesful candidate will work closely with the nursing team to deliver high-quality, patient centred care, supporting individuals with long-term conditions as well as contributing to immunisation and vaccination programmes, health promotion and screening initiatives.
Main duties of the job
- To work in support of the Nursing Team and GPs in delivering high quality care to the Practice population.
- To collaborate effectively with the wider Practice team, ensuring care is relevant, responsive, clinically effective, efficient and delivered to the highest standards.
- To actively contribute to the continuous improvement of quality standards within the Practice.
- To practise in accordance with the NMC Code of Professional Conduct and Scope of Professional Practice, ensuring the highest level of care for the Practice population.

Bexley Group Practice provides care to a registered population of over 14,000 patients, operating from a main site in Welling and a branch site at 24 Station Road, Belvedere. We pride ourselves on being an innovative and patient focused practice, with the best interests of our patients at the heart of everything we do.
Our multidisciplinary clinical team includes GPs, Practice Nurses, Clinical Pharmacists, an Advanced Clinical Practitioner, a Physician Associate and a Phlebotomist. In additon, we are supported by PCN clinicians including a Mental Health Practitioner, Phlebotomst and Physiothereapist. A large and dedicated administrative team provides essential support to the clinical workforce, ensuring the smooth and efficient running of the Practice.

Job responsibilities
- Carry out Chronic Disease Management annual reviews delegated by the Practice Nurse or senior colleague.
- Support the Practice nurse with the management of patients with long-term conditions/chronic disease, using a holistic patient centred approach.
- Assist in and perform routine tasks related to patient care as directed by the Practice Nurse or GP.
- Be able to undertake baseline observations and identify signs of improvement, report any deterioration or concerns e.g. pulse, blood pressure, respiration, oxygen saturation, blood glucose levels, assessing nutrition and hydration, participate in screen programmes and annual reviews for example in patients with learning disabilities.
- administer medications as prescribed and in accordance with NMC standards e.g. via nebuliser or injections using sub-cutaneous or intramuscular route under Patient Specific Directions.
- Following agreed protocols with referral to Practice Nurse or GPs as appropriate assist in the assessment, planning, development implementation and evaluation of programmes to promote health and well-being and prevent adverse effects on health and well-being.
- Deliver opportunistic health promotion using opportunities such as new patient medicals and NHS health checks.
- Document consultations accurately and appropriately, according to NMC guidelines.
- Ensure awareness of statuary and local clinical adult and children safeguarding protection procedures, including systems of referral. Ability to recognise signs and symptoms of abuse/neglect.
- Ensure infection prevention and control guidelines are maintained.
- To be responsible for providing the appropriate nursing evidenced based care of an allocated group of patients.
- Be an accountable professional who is responsible for their actions, working in line with NMC standards of proficiency.
- Recognise and work within the limits of their competency.
- To work as part of the multidisciplinary team using resources efficiently.
- To deliver quality care to patients and to work effectively within the nursing team.
