This role provides high quality patient centred nursing care across our two sites you will run long term condition clinics support health promotion deliver immunisations and carry out core clinical duties you will work both independently and as part of a wider team using your clinical judgement to assess plan and review patient care a valid UK driving licence is needed for travel between sites and for occasional home visits
Main duties of the job
We are looking for an enthusiastic and experienced Practice Nurse to join our friendly, multisite practice. The role involves delivering high quality, evidence based nursing care across all age groups, with a focus on long term condition management, health promotion and preventative care. You will run independent clinics, support QOF delivery, undertake a range of clinical procedures and contribute to the wider clinical team. The post requires strong communication skills, the ability to work autonomously, and a commitment to patient centred care. A UK driving licence is essential for travel between sites and occasional home visits.

Long-Term Conditions
- Run independent clinics for asthma, COPD, diabetes, coronary heart disease, hypertension, anticoagulation and other LTCs.
- Maintain and monitor QOF registers via SystmOne and lead on QOF domains to ensure optimal achievement.
- Review medications, including therapeutic effectiveness and potential interactions, and update repeat prescriptions in line with practice protocols.
- Identify at risk patients and initiate care pathways following NICE and ICB guidance.
Preventative Healthcare and Screening
- Undertake opportunistic screening (BP, BMI, smoking, alcohol use and other risk factors).
- Provide health promotion, including smoking cessation, weight management and lifestyle advice.
- Deliver family planning, cervical cytology, travel health advice and immunisations under PGDs.
Clinical Procedures and Emergencies
- Perform phlebotomy, ECGs, spirometry and wound care.
- Administer vaccines and ensure correct storage, stock rotation and waste disposal.
- Recognise and manage clinical emergencies and participate in resuscitation drills.
Infection Prevention and Control
- Follow infection control policies, antibiotic stewardship and ICB protocols.
- Ensure safe handling and disposal of sharps and biological waste.
Manage needlestick incidents appropriately.
Communication and Record Keeping
- Use clear and sensitive communication, adapting to patient needs and health literacy levels.
- Maintain accurate and contemporaneous records on SystmOne in line with GDPR.
- Participate in audits, meetings, case reviews and guideline development.
- Act as a patient advocate when liaising with external services.
Leadership and Team Development
- Mentor HCAs, Clinical Practitioners, Nurse Associates and student nurses.
- Support shared learning and best practice dissemination.
- Lead on guideline development, audits and commissioning initiatives as required.
Risk Management and Governance
- Identify and report risks, incidents and near misses (SEA).
- Contribute to practice policy and SOP development and review.
Information and Technology
- Use SystmOne effectively for coding, recordkeeping, QOF reporting and data retrieval.
- Run patient searches and analyse audit data for team review.
- Follow GDPR, confidentiality and Freedom of Information requirements.
- Use evidence based online resources to support care.
