Main Purpose of the Post

To provide a comprehensive range of nursing care within the GP practice, including the development, implementation, and evaluation of care and treatment programmes for patients with long-term conditions.

Deliver general clinical care and chronic disease management reviews, working independently within practice protocols and the Year of Care model.

Maintain strict confidentiality of all patient information at all times.

Main duties of the job

The main duties of this role include general practice nursing responsibilities, including (but not limited to):

Chronic Disease Management

Wound Care

Immunisations (routine and travel for adults and children)

Cervical Screening

Injections

Ear checks and care

The surgery is a proud provider of the Year of Care initiative, which focuses on delivering effective, individualised care to patients with longterm conditions. The postholder will play an instrumental role in delivering this service to our patient population.

About us

Trinity Medical Centre is a friendly and forward-thinking GP Surgery with an approximate list size of 6300 patients. We are looking for an enthusiastic and dedicated Practice Nurse to join our team.

We aim to provide safe, effective, and easily accessible healthcare services to all of our patients within a friendly and confidential environment. Our care is delivered by a dedicated, compassionate team working collaboratively with colleagues across health and social care.

We are a busy practice located in the heart of South Tyneside. Our team consists of 3 GP Partners, 1 Salaried GP, a Nurse Practitioner, 1 Practice Nurse, 1 Healthcare Assistant, and a full complement of administrative staff. We are now looking to recruit a Practice Nurse for 25 hours per week to enhance our current nursing team and help us continue providing exemplary, patientfocused, highquality care.

Prime Responsibilities

The post holder is responsible for:

Delivering general practice nursing care to the practice population.

Providing evidencebased care for patients with longterm conditions.

Contributing to preventative care, health promotion, and patient education.

Working collaboratively with the wider healthcare team to meet patient needs, and supporting the implementation of policies, guidelines and procedures.

Clinical practice

Assess, plan, implement and evaluate treatment programmes that promote health and wellbeing.

Provide individualised care for patients with longterm conditions.

Offer clinical advice within own scope of practice.

Support patients in medication adherence and safety.

Prioritise urgent or emergency problems and initiate appropriate care.

Deliver cervical cytology, womens health support, and family planning advice.

Recognise and refer mental health concerns appropriately.

Provide adult and child immunisations, including travel vaccinations.

Deliver wound care.

Key Duties

Provide holistic assessments and health promotion advice.

Run chronic disease clinics including Year of Care reviews.

Complete asthma, COPD, CHD, stroke, hypertension and diabetes reviews.

Undertake contraception and HRT/menopause reviews.

Administer vaccinations and injections.

Assist with minor procedures and IUCD fittings.

Perform ECGs, venepuncture and prepare laboratory samples.

Undertake vaginal swabs and liaise with clinicians on treatment.

Supervise and support the Healthcare Assistant.

Delivering a quality service

Work within NMC standards and maintain professional boundaries.

Ensure accurate and contemporaneous clinical record keeping.

Participate in audit, quality improvement and governance processes.

Support and learn from significant events and complaints.

Contribute to shared learning across the practice.

Review effectiveness of care and support evaluation of services.

Management of risk

Assess and manage clinical risk in all areas of responsibility.

Maintain safe storage and disposal of vaccines and medications.

Adhere to infectioncontrol requirements and coldchain standards.

Complete all mandatory training to promote safe practice.

Knowledge, Skills, Training and Experience

Registered General Nurse with valid NMC registration.

Knowledge of the structure and function of General Practice.

Understanding of the GP Contract and Quality and Outcomes Framework.

Experience in chronic disease management (preferred).

Cervical cytology qualification (or willingness to undertake).

IT literacy and experience with clinical systems (e.g., EMIS).
