Job summary
We are seeking an enthusiastic Clinical Pharmacist to work
across our two PCN practices, on a Full or Part-time basis.
We are a friendly surgery with
a list size of 15,000 patients, over two sites, Wootton Vale, (MK43 9JJ), and branch site Shortstown Surgery, (MK42 0GS). We have good
appointment access, which means happy patients, and a strong teamwork
ethos that includes an established clinical pharmacy team, GPs, Nurses,
Physician Associates, & a clinical liaison team who look after the clinical
staff.
We support staff development and wellbeing, & we have several
members of staff on diploma programs or clinical leads in BLMK. We will support & encourage you to build on
your LTC skills to attain diploma status. The ideal candidate willhave achieved a
non-medical prescribing qualification and CPPE however, we are happy to support the right candidate to attain both, if required.
We embrace IT & automation & often pilot new innovations. Soon,
we will become an accredited GP training practice.
Training on local services will be provided.
Both practices are
based on the outskirts of Bedford town centre, and have easy access & free on site car
parking.
Both practices share the same clinical & management
teams, & are active members of our PCN, (Caritas). This role is pivotal to improving the
quality of care & operational efficiencies & so, requires motivation & passion to deliver an excellent service within general practice.

Main duties of the job
The post holder will work within their clinical competencies as
part of a multi-disciplinary team to provide expertise in the following areas:
- Clinical medicines management,
- Provide face to face/remote structured
medication reviews,
- Management of long-term conditions Asthma/COPD,
CVD, Diabetes, Dementia/Frailty all desirable
- Medicines optimisation, review of
monitoring needs and blood interpretation
- Management of medicines on transfer of
care and systems for safer prescribing,
- Manage prescription requests including
authorisations and reauthorisations,
- Medicines safety and quality
improvement, incorporating the use of national tools,
- Ensure both the public health and
social care needs of patients within the GP practices.
- Regular audits to improve and
influence prescribing across the practice
About us
We are one practice split across two sites. The main site is at Wootton, (MK43 9JJ), and a branch site at Shortstown, (MK42 0GS).
Both practices share the
same clinical & management teams and all practice staff are expected to work from both surgery locations.
We understand and value the expertise of
our clinical pharmacists. Our ethos is to embrace new ways of working &
workforce development & we have been involved in numerous early adopter
& NHS pilots developing roles, including the first wave of Clinical Pharmacists in primary care in 2016. We also work closely with our ICB in
supporting quality & development in areas such as IT & medicines management.
The practices have a combined
list size of 15,000. The two practice lists vary greatly in terms of
demographic & depravation scores, thus providing a diverse, interesting mix
of clinical challenge & will therefore appeal to clinicians looking to
broaden their range of exposure & experience.

As an experienced clinical pharmacist,
you will provide care for patients, undertaking structured medication reviews, long
term condition clinics, ensuring medicines optimisation and offering medicine
information services to patients and clinicians. You would be required to work
collaboratively with our healthcare team that includes pharmacists, GPs,
specialist nurses as well as AHPs.
You will be providing care that
you will deliver autonomously but, from within a supportive clinical team. You will be assessing patients where
applicable, devising, implementing, and agreeing management plans, and
following approved protocols as appropriate.
When necessary, you will make onward referrals to other agencies and
members of multidisciplinary teams.
Training on local services will be provided.
A minimum of 2 years post graduate
experience in pharmacy, as demonstrated within a practice portfolio is required.
