Pharmacist ICO Health Group ICO Health Group is seeking a motivated and experienced Pharmacist to join our progressive and supportive general practice team in South London. This is an exciting opportunity to play a vital role within a forward-thinking primary care network (PCN) focused on innovative patient care and medicines optimisation.
About the Role As a key member of our multi-disciplinary team, you will work within your clinical competencies to provide expert clinical medicines management. Your responsibilities will include:
- Conducting face-to-face and telephone structured medication reviews
- Managing long-term conditions and repeat prescription authorisations
- Overseeing medicines management during patient transitions of care
- Implementing systems to support safer prescribing practices
- Leading quality improvement initiatives and clinical audits, including managing aspects of the Quality and Outcomes Framework (QOF)
- Supporting public health and social care needs within the PCN
You will champion a positive culture of medicines optimisation, helping ICO Health Group and the wider PCN to achieve local and national medicines-related objectives. This role offers a unique chance to contribute to the transformation of clinical pharmacy services in primary care, aligned with national healthcare strategies such as the Five Year Forward View and GP Forward View.
