Job summary
We are looking for a Practice Nurse with Asthma experience to join our diverse team. The ideal
candidate will be dynamic and highly motivated with general practice
experience. You will be NMC registered with strong interpersonal and
communication skills and it is imperative you are a team player.
x1 vacancy for 16 hours per week working acrossWednesday PM, Thursday PM and all day Friday. Unfortunately the days are non-negotiable due to clinical room allocation.The successful candidate will be expected to work across the two sites.
Main duties of the job
The ideal candidate will work as an integral part of the Primary Health Care Team. Working autonomously with our patients within the surgery and our practice community. This role is varied and covers all aspects of general practice nursing duties. The ideal candidates must be a team player able to adapt to changing work practices and be flexible to cover sickness and holidays. The ideal candidates must have attention to detail as accuracy is a key requirement of the role. You must also have excellent communication skills both verbal and written.
Main duties of the job
- Womens health (cervical cytology,
contraception, etc)
- Wound care and dressings
- Chronic disease management (Asthma in particular)
- Routine and child immunisations and vaccines
- Travel vaccinations
- Requesting pathology tests and
processing the results, advising patients accordingly
- Support the delivery and achievement of QOF targets, including data collection, clinical recall, and evidence-based care
- Knowledge and experience of working within the framework of Locally
Commissioned Services and Direct Enhanced Services

- Responsible for nurse led Family Planning Clinic
- Responsible for nurse led Travel Clinic including registered and unregistered patients
- Responsible for nurse led Child Immunisation Clinic
- Responsible for nurse led Sexual Health Clinic including cervical cytology
- Responsible for nurse led Dressing Clinic
- Management of chronic diseases such as Asthma, COPD, Diabetes, Hypertension and Coronary heart disease
- Health promotion and lifestyle advice
- Other routine nursing duties; BP, single injections, phlebotomy, ECG, Goserelin injections, Urine dips
- Consulting will include face to face, electronically via accurx and telephone consultations
- Support the seasonal flu campaign, including patient recalls, vaccine administration, data recording
- General administration including patient queries and patient requests, statutory reports, medical attendance reports.
- Review and maintain accurate records (including practice notes, computer and patient hand-held records etc) according to the Guidelines for records and record keeping (NMC, 2015)
- Ordering of stock and keeping clinical rooms stocked
- Support the delivery and achievement of QOF targets, including data collection, clinical recall, and evidence-based care
- Observe and comply with all appropriate statutory requirements in relation to Child and Adult Safeguarding and to be familiar with the reporting process should this be required
- Attendance at clinical meetings
- Any other delegated duties appropriate to the post
