We are recruiting 2 wte Advanced Clinical or Nurse Practitioners to join our team and are looking for forward-thinking, hard-working candidates who would thrive in a busy but inclusive environment, like ours. Many of our staff have worked at the practice for a long time because they are in a positive, supportive environment - where everyone's contribution is respected.

As a practice we are developing a partnership with the local Foundation Trust, County Durham and Darlington NHS FT. A unique aspect of this role will involve working with our hospital colleagues, particularly in Community Services, to improve our offer to patients.

Main duties of the job

The Advanced Clinical Practitioner will act autonomously within their professional boundaries. They will demonstrate safe critical thinking in the clinical decision-making process and provide expert care, from initial history taking, clinical assessment, diagnosis, initiating treatment and evaluating care. They will work collaboratively as part of the practice multidisciplinary team, delivering care within their scope of practice and agreed acceptable limits of practice, to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence. They will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner, supporting the delivery of policy and procedures.

Job responsibilities

Clinical Practice

Assess patients who present with previously undifferentiated, undiagnosed problems, taking responsibility for their health assessment and physical examination.

Make differential diagnosis using decision making and problem solving skills.

Plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.

Make professional autonomous decisions for which they are accountable.

Order necessary investigations and provide treatment and care, both individually, as part of a team and through direct referral to other primary and secondary agencies.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

Plan, implement and evaluate the chronic disease management programmes in line with current evidence based best practice and the GMS contract, reducing unnecessary hospital admissions and promote the preventative aspect of nursing practice.

Prescribe appropriately as an Independent Non-Medical Prescriber, using the practice formulary. Prescribing and reviewing medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies.

Fully participate in the clinical administration within the practice, signing prescriptions, reviewing medication, EPS, reviewing patient letters/discharges summaries and other clinical correspondence.

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.

Review and process laboratory results.

Fully document all clinical and appropriate information within patients computer records from both face to face and telephone consultations and all other appropriate instances together with appropriate read coding.

Work with patients and their carers in order to support compliance with and adherence to prescribed treatments.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.

Support and manage health needs of women presenting for family planning or sexual health consultation.

Ensures awareness of statutory and local clinical protection procedures, including systems of referral.

Participate in vaccination and immunisation programmes for both adults and children.

Work as part of a multi-disciplinary team to deliver safe and high quality care.

Care Management

Communicate confidential and sensitive information to patients, relatives and carers in relation to their condition.

Provide highly specialised care advice beyond the scope of normal practice e.g. care treatment plans, making referrals as appropriate (including fast track referrals).

Contribute to local and national practice targets clinical remit e.g. QOF, prescribing incentive scheme, National benchmarking.

To monitor and meet care outcomes against standards and recommend or initiate changes as necessary.

Health Improvement

Management of health screening programmes and ensures other team members are aware of health priorities and screening objectives and programmes.

Innovation in practice to meet ongoing demands on the practices delivery of care.

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures

Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate.
