Job summary

We are a friendly, supportive GP surgery looking for an experienced and enthusiastic Practice Nurse to join our busy practice team. We are currently a team of 5 Practice nurses and 3 Health Care Assistants and are looking for a full time nurse to join our team.

Main duties of the job

RGN with NMC registration. Experience in managing chronic conditions. Specific nursing skills / qualifications including cervical cytology, wound care, immunisation. Commitment to continuing professional development. Excellent communication and interpersonal skills. Ability to work autonomously and collaboratively within a team. To be able to determine workload priorities. Able to work under pressure in an ever changing environment. Flexibility of working hours that may include extended hours.

We currently have 20 GPs, 5 practice nurses, 3 HCAs, and an extensive team of support staff. We are friendly and collaborative in our approach, aiming to be a happy, high functioning team.

Practice Nurse Accountable to : GP Partners Reporting to : Senior Practice Nurse Job Purpose : The post holder will work as a practitioner, assisting in providing nursing services to meet the needs of the practice population, including treatment room work, health promotion, sexual health services, immunisation programmes and management of long term conditions within a defined role. They will be responsible for raising awareness of health and social wellbeing, enabling people to improve their own health and work collaboratively to tackle the health of the practice population. Part of a multidisciplinary team of Nurses, GPs and associated health professionals and admin staff. To provide an environment for the patient which is welcoming, non-judgmental and respects their circumstances, feelings and rights. To work within the culture of NHS Englands 6Cs of Care, Compassion, Competence , Communication, Courage and Commitment. Role responsibilities : Undertake appropriate treatment room duties, management of long term conditions, screening programmes, immunisation programmes and sexual health services within their level of competency and with adequate and recognised training. Ensure accurate completion of all necessary documentation associated with a patients healthcare. Perform home visits where appropriate. Responsibility for reviewing and acting on own sample results or those referred onto you from another nurse or associated healthcare professional, or referring to a more senior clinician when appropriate. Providing nursing treatments to patients in participation with general practitioners or independently, meeting agreed protocols. Support and promote the development of team working to ensure a high standard of nursing care within the treatment area. Adhere and work to defined polices and protocols, including health and safety, equality and diversity, and confidentiality within the practice Assist in the ordering and maintaining of stock as delegated by the senior nurse. Actively be involved in the recall of patients with long term conditions for a minimum annual review, immunisation , smear and QoF targets within the limitation of own skills. Support the lead nurse in the education, training and ongoing development of new nurses and healthcare assistants that form the nursing team. Be aware of national standards of infection control and good practice guidelines and active observation of current working practices across the practice ensuring that procedures are followed. Actively identifying, reporting and correction of health and safety hazards. Responsible for the safe use of all diagnostic and treatment equipment, and alert other team members to issues of quality or risk. Attend practice meetings whenever possible and those that are mandatory. Maintain competency and develop new skills as decided by service requirements. Communicate, refer and liaise effectively with other agencies and staff regarding patients well being, with appropriate regard to confidentiality. Effectively manage own time, workload and resources. Professional Development : The post holder will be accountable for their own professional actions in an unsupervised manner but supported by the Senior nurse and GPs and working in accordance with the NMC Code of Conduct. Take responsibility for their own development and continued education by attendance of courses or study days as deemed useful or necessary within the role by the Business manager, Senior nurse or GPs.
