The Links Medical Practice is a GP Practice that operates from 2 sites in the Borough of Bromley.
An excellent opportunity has arisen for a highly motivated individual to join our friendly team.
The position is permanent, part-time with a flexible shift pattern. Hours of work range between 7:45am - 6:45pm Monday to Friday.
The applicant(s) will be based at either site - Mottingham site - 198 Court Farm Road, London, SE9 4JS andDownham site - 27 Brook Lane, Bromley, BR1 4PX
Main duties of the job
We are looking for a proactive and well-organised Care Coordinator to manage the recall process for patients with long-term health conditions. This role involves ensuring patients are invited for their regular reviews on time and that all elements of their care are completed with the appropriate clinicians. The Care Coordinator will also support accurate and timely patient registrations as part of their responsibilities.
Previous experience in this role is desirable.
About us
The practice is managed by 4 GP Partners and the Practice Business Manager.
Additional team members are a further 4 GPs, 3 Physicians Associates and 6 members of the nursing team. There are approximately 15 administrative and reception team members. We also actively collaborate with 2 other practices as a Primary Care Network team.
The practice values each other and the skills individuals contribute to the practice both in terms of daily work schedule and the delivering of patient care.

This role plays a key part in supporting patients with long-term health conditions by coordinating their regular reviews and ensuring all aspects of their care are delivered by the appropriate clinicians.
