Job summary
We are
looking to recruit an experienced, dynamic primary care pharmacist, independent
prescribing qualification is desirable but not essential although should be
willing to work towards this. The role will be based in our practice alongside
our clinicians including pharmacists already in surgery deployed from our PCN.
We are
looking for a practice pharmacist to work closely with the team to ensure we
deliver an effective, efficient and quality medicines management service to our
patients and our team.
Main duties of the job
The
Pharmacist will undertake the following tasks.
Medication
reviews Face to face, remote and e-consultations) and proactively manage
patients (some with complex polypharmacy). This will include patient education,
prescribing and monitoring as per guidelines and within professional
competencies.
Work
closely with the wider team, advising and supporting on medicine management
issues.
Clinical
input regarding the supporting GPs with medication titration.
Undertake
prescribing audits.
Ensure
that the practice is encouraged to promote cost effective prescribing.
Ensure
medicines are prescribed and monitored as per the most current guidelines.
Ensure
that the practice systems for drug safety and drug safety monitoring are
adhered to at all times. This work will include regular safety audits.
Share
knowledge and learning with the wider teams.
Attend
regular practice meetings.
Represent
the practice at outside events.
Contribute
to achieving high IIF/ QOF targets.
Undertake
medication reviews for housebound/care home patients as needed.
Take an
active role to help/ support practice initiatives.

medicines following hospital discharge and work with patients and the community
pharmacists to ensure that the patients receive their medication post
discharge.

Support
long term condition reviews via face to face and telephone appointments.
Contribute
to reductions in medicine related hospital admissions and re-admissions by
supporting patients to get the best outcomes from their medicines.
Identify
and address medicine related issues.
Assist
in managing patients and healthcare professional medicine queries.
Increase
patient quality and patient safety of prescribing through mechanisms such as
audit and PDSA cycles. The share the findings with the wider team.
Contribute

- To be responsible for own professional development and maintain competence within own clinical field.
