Our friendly and dynamic
rural GP training/dispensing Practice is looking for an Experienced Practice
Nurse to join our team.
This is an exciting
opportunity to join our established Nurse Team and provide excellent care for
patients. The role will be dual sited within Godolphin Health's Praze and
Connor Downs GP practices.
We welcome applications from
experienced Practice Nurses with relevant chronic disease management experience
but would also encourage applicants from those wishing to move into a treatment
room Nurse role as a first step into general practice.

To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team.
About us
We are a training practice looking
after a list of 9025 patients over two sites, dispensing to 65% of them. The 3
partners are supported by a growing fantastic multidisciplinary team comprising
5 salaried GPs, 1 ANP, 1 Paramedic, 2 Practice Nurses, a HCA and a Phlebotomist. In addition, we have our GP trainees. Through
our Primary Care Network, we share 3 Mental
Health Practitioners, a first contact Physiotherapist, care home Paramedic and
a share of a Pharmacist and 3 Pharmacy technicians. We are all a friendly and supportive bunch!
We run a total triage system via
Klinik.We provideEnhanced Accessto General Practice
services on a shared rota.
We have a weekly MDT with the whole
clinical team which is a great time to come together for patient care and
personal learning. We hold a three-monthly protected half-day clinical
governance/educational meeting for all staff.

The main duties of the job
To provide and
maintain a high standard of nursing care for patients as well as
providing nursing assistance to the doctors and other members of the primary
healthcare team. The duties will
include:
- All
tasks normally undertaken by an experienced RGN and in addition any rules agreed
between the nurse and doctors as appropriate.
- Registering
pathology test for example urine culture swabs
- Interest
in Asthma, COPD and learning difficulties
- Chaperoning
and assisting patients who are being examined by clinician
- Attend
and participate in the practices MDT
- To
manage patients with chronic disease running specific clinic sessions and
maintaining robust call and recall systems
- Participate
in national screening work such a cervical cytology and deliver the
national immunisation vaccine schedules
- Administration
to maintain accurate patient records and enter onto EMIS using agreed Read
Code and templates
- Ensure
accurate entries
- Maintain
stock of waiting room leaflets health promotion leaflets
- Offer
advice about childhood and adult vaccinations and ensure vaccines are
administered under patient group directions
- Monitor
vaccinations to maintain stock levels
- Running
well person clinics and health promotion programs to provide a holistic
assessment and lifestyle advice on diet, smoking, alcohol intake and
exercise
- Able
to document consultations according to the NMC guidelines
- Ensuring
awareness of statutory and local clinical protection procedures including
systems of referral ability to recognise signs and symptoms of child abuse
- Follow
agreed clinical protocols with referral to GPs as appropriate

- Understand the requirement for PGDs is PSDs and associated policies.
- Experience working with a nursing team with a mixed patient care group adults and children.
- Experience working autonomously.
- Experience working as a practice nurse or community nurse.
- Experience of infection prevention and control measures.
Desirable
- Experience of managing elderly frail patients.
- Experience a quality initiatives and audit.
- Experience of working in primary care.
- Experience working as a practice or community nurse.
- Family-planning qualification.
- Mentor or teaching qualification.
- COPD, asthma qualification and Certified smear taker.
- Any additional related study modules for chronic disease management and long-term conditions.
