Job summary
We have an exciting new opportunity for a Patient Care Navigator to join our busy, happy Team here at Godolphin Health. We are looking for an enthusiastic and organised person to carry out this role.
The post holder will be responsible for undertaking a range of tasks all related to patient information. You will require excellent customer service skills as your role will involve direct face to face contact with patients. Tasks also involve patient contact via telephone and the use of our various electronic medical information systems.

Main duties of the job
To provide the Team with an outstanding patient reception and support service.
- Greeting and attending to patients in person and over the phone
- Professionally assisting doctors, staff, visitors, and patients
- Answering all phone calls in a professional and courteous manner
- Maintaining appointments and providing direction to people in person and on the phone
- Performing associated clerical work and disseminating information to the relevant medical Team members. Competently using the associated software.
- Scheduling and reorganising appointments.
- Assisting patients with completing necessary forms and documentation, as well as ensuring that all information acquired is accurate and current
ESSENTIAL SKILLS
- GP Surgery Experience
- Good IT Skills
- Excellent Customer Service Skills
- Able to work on a Monday and a Friday
- Ability to drive. You will be on a rota covering 2 surgery sites, that are 4.5 miles away from each other. However it is very unlikely that you will be asked to move in between Practices unless there is an urgent manning problem.
DESIRABLE SKILLS
- Navigate the Medical Information Systems EMIS, DOCMAN & KLINIC

We currently offer a fantastic multidisciplinary team of Clinicians that are led by 3 Partners. These are supported by experienced Managers, dispensers, administration and patient care navigation personnel.
We work from 2 large purpose-built surgeries, 1 at Praze an Beeble and the other at Connor Downs. They are separated by 4.5 miles of rural countryside.
Regular training events keep our Team up to date with their roles and we keep morale high by organising well supported social events.
Uniform and onsite parking is provided.
We are also a member of a Cycle to Work Scheme - Cyclescheme.
