Our Practice is part of CWW Primary Care Network and works collaboratively with the other 5 GP Practices within this network, offering additional support and access to a wider healthcare community overall.
Main duties of the job
You will be a Registered Nurse - motivated, proactive, committed with excellent communication skills and will be responsible for the delivery of basic practice nursing services care to the Practice population. Supported by senior nurses within the Practice, you will deliver care within the boundaries of the role, focusing upon supporting patients to be healthy, monitoring of long term conditions, health prevention and screening activities. You will work collaboratively with the General Practice team to meet the needs of the patients and providing nurse leadership as required. You will work
independently under the leadership of our Registered Nurses, supporting with
all aspects of nursing care and improving on the safety and quality of patient
care at every opportunity.
