Job summary

Castleton Health Centre is looking for an enthusiastic and proactive Practice Nurse to join our friendly team. We value teamwork, mutual support and a friendly working environment. We are looking for NMC registered professional, with General Practice Nurse experience.

Main duties of the job

The post holder will be an experienced Practice Nurse, who acting within their professional boundaries, will provide high quality care for patients. The role includes the typical practice nurse duties including: diabetic & respiratory care (asthma & COPD), smears, immunisation, contraception, health reviews, etc.

The individual will provide nursing service/treatments to patients independently or in participation with general practitioners working within the boundaries of the NMC Code of Conduct to agreed practice standards and protocols.

Key Responsibilities

  • Deliver a wide range of nursing services, including immunisations, and cervical screening
  • Lead and support the management of chronic diseases such as asthma, COPD, diabetes, and hypertension through structured clinics and patient reviews
  • Undertake chronic disease monitoring, including observations, blood tests, and interpretation of results within scope of practice
  • Develop, implement, and review individualised care plans for patients with long-term conditions
  • Provide education and self-management support to patients, empowering them to take an active role in managing their health
  • Conduct NHS health checks, screening programmes, and preventative care initiatives
  • Promote healthy lifestyles, including advice on smoking cessation, diet, exercise, and weight management
  • Administer vaccinations in line with national immunisation schedules
  • Maintain accurate and up-to-date clinical records in accordance with practice policies
  • Work collaboratively with GPs and the wider multidisciplinary team to ensure coordinated care

    • Maintain valid registration with the Nursing and Midwifery Council (NMC)
    • Work in accordance with the NMC Code and professional guidelines
    • Ensure patient confidentiality and compliance with data protection regulations

    • Participate in clinical audits, quality improvement, and ongoing training/CPD
    Skills and Attributes
