Job summary
Town Gate Practice is looking to recruit an experienced Advance Nurse Practitioner to join our friendly and supportive team. Preferably with a background in primary care and an independent prescriber, you will be responsible for the clinical assessment of our patients, undertaking consultations as an autonomous practitioner, using your advanced clinical practice skills and clinical judgement to diagnose, treat and refer patients.
Main duties of the job
This role will be primarily face to face same day care and include a share of home
visits where appropriate.

are located in Chepstow in the beautiful Wye Valley within easy access of
Cardiff, Bristol and Gloucester. Our Chepstow
site is within the Community Hospital offering direct links with other service facilities.
The newly refurbished branch surgery is located in Sedbury, less than a mile
away.
We
are a high QIF achieving dispensing and training practice providing care to
over 9000 patients. We have a supportive
and friendly team of four partners, three salaried doctors, two pharmacists,
three practice nurses, three health care assistants and a helpful admin support
team. The clinical system used is Emis.
We are looking for patient focused forward thinking applicants who will enhance our current team. Ideally to work between 24 and 34 hours a week over three or four days to include Monday and Thursday.
The successful
candidate will be rewarded with competitive pay, pro rata bank holiday allowance
