Job summary
The Clinical Team Lead is a senior clinical
role within the service, providing professional leadership, supervision, and
support to the Home Visiting Service clinical team. The post-holder will work
closely with the Clinical Operational Lead and Deputy Head of Patient Services to
ensure the safe, effective, and consistent delivery of the service.
Main duties of the job
The Clinical Team Lead will:
- Provide line management, senior clinical leadership, mentoring, and professional support to clinicians within the Home Visiting Service.
- Offer senior clinical oversight and provide safe triage, assessment, and complex decision-making.
- Work collaboratively with the Clinical Educational Lead and Clinical Operational Lead to ensure high standards of clinical quality, governance, and continuous service improvement.
- Promote professional development across the multidisciplinary team, leading on clinical learning, audit and best practice.
- Contribute to strategic service development and innovation across Sutton PCNs.

Sutton has a population of approximately 200,000 residents registered across 21 GP practices within four Primary Care Networks (Carshalton, Cheam & South Sutton, Central Sutton, and Wallington). Each PCN serves a population of approximately 50,000 patients.

The Home Visiting Service (HVS) plays a vital role in supporting practices by managing acute presentations and providing continuity of care for frail and housebound patients, helping to reduce unnecessary hospital admissions.

Job responsibilities
Clinical Leadership and Support
- Provide line management and clinical guidance to Home Visiting Service clinicians, supporting complex decision-making and patient management.
- Support clinicians with clinical triage and referrals, ensuring appropriate patient selection and escalation.
- Provide triage cover to maintain service continuity.
- Review and appropriately challenge referrals made outside the HVS inclusion criteria, ensuring safe, appropriate patient pathways.
- Provide case-based learning and feedback following complex cases, incidents or audit findings.
- Support the integration of clinical governance principles into daily practice.
- Participate in and lead clinical and non-clinical audits to ensure safety, quality and effectiveness of the HVS.
- Provide clinical supervision and mentoring for both substantive and locum clinicians.
- Identify training needs and develop learning events to support professional development across the team.
Collaboration and Teamworking
- Work closely with the Clinical Operations Lead to align clinical delivery with operational processes.
- Contribute clinical expertise to service development, quality assurance, and performance monitoring.
- Support the Proactive care team by undertaking assessments of individuals referred to the service as required and develop care plans with the patients/family / carers as appropriate
- Ensure Universal care Plans are completed and uploaded for these individuals
- Support the with implementation of clinical governance frameworks and protocols.
- Collaborate with Medical Directors, Clinical Leads l Directors, Practice Leads, and ARRS roles to support integrated care delivery.
- Participate in multidisciplinary team meetings, promoting collaboration and shared learning.
- Liaise effectively with GPs and GP practices, community teams, urgent care providers, and social care partners to ensure seamless patient care.

- Assess, diagnose, and manage patients at home or in primary care settings, using advanced clinical reasoning and evidence-based practice.
- Provide continuity of care through follow-up and safety-netting where required.
- Prescribe or recommend medication where appropriate and within scope of practice.
- Contribute to reducing unnecessary hospital admissions and improving patient outcomes.
- Maintain high-quality clinical documentation and data integrity.

Administrative and Governance Support
