The Virtual Community Project at SPCNs (Sutton
Primary Care Networks) is an innovative initiative focused on delivering
tech-enabled care that is both proactive and relational. This project, funded
by the Health Foundation, aims to address the challenges of loneliness and
diabetes among registered patients in Sutton, aligning with the recent NHS
10-Year Plan to deliver care closer to home and utilise technology. Through
weekly virtual sessions over a twelve-week period, facilitated on the Togetherly
platform, the project will foster connection, provide support, and encourage
self-management within a supportive online environment
Main duties of the job
- Session Facilitation: Lead and facilitate weekly 12-week virtual group sessions on the Togetherly platform, ensuring a welcoming, inclusive, and safe atmosphere for all participants.
- Content Delivery: Plan and develop session in line with the interest and needs of the group.
- Community Building: Foster positive interactions, encourage peer support, and help members build connections within the virtual community. Employ effective virtual facilitation techniques specific to the Togetherly platform to maximise participant engagement and ensure active involvement in sessions.
- Feedback & Reporting: Collect feedback from participants (as required by the Project Lead) and provide regular reports on session progress, challenges, and successes.
- Safeguarding: Adhere to all SPCNs safeguarding policies and procedures, escalating any concerns to the Project Lead immediately.
About us
The Virtual Community Project Facilitator will be responsible for
leading and supporting weekly virtual sessions for community members. The Bank Facilitator
will create an engaging, safe, and supportive online environment on the Togetherly
platform, guiding discussions, ensure the space remains safe, fostering
peer-to-peer interaction, and ensuring that participants gain valuable insights
and support related to loneliness and/or diabetes management. For the diabetes element of the project a
registered nurse will also facilitate/support the group. Facilitators will be
given 3 hours per group which includes; ! hour to plan the session; 1 hour to
deliver the session and 1 hour to follow up participants post session / provide
feedback to the project team.

Qualifications

- Experience in group facilitation, community building, or leading online discussions.
- Safeguarding Level 2 training, or willingness to undertake it upon appointment.
- Strong empathetic/interpersonal skills, with the ability to build rapport within virtual setting.
- Understanding of the challenges associated with loneliness and/or living with diabetes
- A good grasp of using IT and learning new software
- Experience of working in the NHS/Primary Care
- Excellent verbal communication skills.
- Ability to maintain professional boundaries and confidentiality.
- Ability to work independently and as part of a virtual team.
