Job summary
We have an opportunity for Health and Well-being Coach to join the team at our practice sites in and around Sutton. A great opportunity to work in an innovative and fast expanding Primary Care Networks in Sutton Surrey. Sutton Primary Care Networks (PCN) was formed from the merger of 23 high achieving GP practices and our services are offered from different GP Practices in and around Sutton, Cheam, Wallington and Carshalton. We aim to provide the best quality care for the local community.
We are looking for Health and Well-being Coach to develop collaborative relationships and work in partnership with health, social care, and community and voluntary sector providers and multi-disciplinary teams to holistically support patients wider health and well-being, public health, and contributing to the reduction of health inequalities; provide education and specialist expertise to PCN staff, supporting them to improve their skills and understanding of personalised care, behavioural approaches and ensuring consistency in the follow up of peoples goals with MDT input; work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN, in accordance with the needs, priorities and any urgent support required by individuals.
Main duties of the job

We are looking for dynamic and enthusiastic coaches, able to work full time or job share who will support people to take pro-active steps to improve the way they manage their physical and mental health conditions, based on what matters to them, to improve their health outcomes and quality of life. The successful candidates will be kind, reflective and self-aware, enjoying working with a wide range of people. They will have good communication and negotiation skills and able to provide a high quality, non-judgemental service.
About us
Primary care networks (PCNs) form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; to manage financial and estates pressures; to provide a wider range of services to patients and to more easily integrate with the wider health and care system. While GP practices have been finding different ways of working together over many years for example in super-partnerships, federations, clusters and networks the NHS long-term plan and the new five-year framework for the GP contract, published in January 2019, put a more formal structure around this way of working, but without creating new statutory bodies. Since 1 July 2019, all except a handful of GP practices in England have come together in around 1,300 geographical networks covering populations of approximately 3050,000 patients. This size is consistent with the size of primary care homes, which exist in many places in the country, but much smaller than most GP federations. Around 50 networks, usually in very rural areas, will cover a population of less than 30,000, but most are bigger than 50,000.
