Job summary

An exciting opportunity has arisen for a motivated and compassionate Healthcare Assistant (Band 4) to join our award-winning, training and research-active practice.

Working within a forward-thinking, multidisciplinary team, you will play a key role in delivering high-quality, patient-centred care to our diverse population. You will support the nursing and clinical team in the delivery of proactive, preventative healthcare, with a strong focus on reducing health inequalities and improving outcomes.

This role is ideal for an experienced HCA looking to develop their clinical skills and take on an enhanced scope of practice within a supportive and innovative environment.

You will work closely with GPs, Practice Nurses, and the wider multidisciplinary team to deliver a range of clinical and administrative duties. You will contribute to our proactive care model, including enhanced NHS Health Checks, chronic disease monitoring, and targeted work to improve screening and immunisation uptake.

You will also support patients with complex health and psychosocial needs, working collaboratively with our in-house Integrated Health Worker and neighbourhood teams.

Main duties of the job

Clinical duties

  • Undertake clinical observations and procedures including BP, BMI, venepuncture, ECGs, and urinalysis
  • Support delivery of NHS Health Checks and screening programmes
  • Assist with long-term condition monitoring and patient recall
  • Support immunisation and vaccination clinics (including childrens, where trained)
  • Perform wound care and dressings within competency
  • Act as chaperone when required
  • Identify and escalate any clinical concerns to a registered clinician

Patient care and health promotion

  • Provide health promotion and lifestyle advice (e.g. smoking cessation, weight management)
  • Support uptake of screening and preventative programmes
  • Deliver compassionate, person-centred care to a diverse population

Administrative and organisational duties

  • Maintain accurate clinical records
  • Support recall systems, QOF, and quality improvement activity
  • Ensure clinical rooms are clean, stocked, and prepared
  • Adhere to infection prevention and control procedures

Team working and development

  • Work collaboratively within the multidisciplinary team
  • Participate in training, meetings, and quality improvement initiatives
  • Contribute to a positive learning and development culture

Professional responsibilities
