Job summary
We are
looking for a motivated and committed Clinical Pharmacist to join our
friendly, forward-thinking practice team. Working across our two sites, you
will support patients and staff with medicines optimisation, medication reviews
and prescribing queries. You will work closely with GPs, other pharmacists,
nurses and wider colleagues to deliver safe, evidence-based care and improve
health outcomes for our diverse patient population.

Main duties of the job
- Conduct
structured medication reviews, including for patients in high-risk groups
such as frailty, polypharmacy, and high-risk medications.
- Manage
complex polypharmacy and reduce inappropriate prescribing where possible.
- Support
management of chronic diseases through safe prescribing and monitoring.
- Manage
the repeat prescription system, including reauthorisation, requesting
blood tests, and responding to prescription queries.
- Complete
medicines reconciliation following hospital discharge and transfers of
care, liaising with patients and community pharmacists to ensure
continuity of supply.
- Support
the development and implementation of cost-effective prescribing
strategies.
- Provide
leadership in medicines optimisation, medicine safety projects, audits and
quality improvement initiatives (including QOF work).
- Implement
a medication review system to ensure patients are recalled for monitoring
and reviews throughout the year.
- Contribute
to practice meetings, sharing expertise and supporting service
development.
- Refer
to senior GPs when appropriate and involve patients in shared
decision-making about their medicines.
