Job summary
An exciting opportunity has arisen for
an experienced Advanced Nurse Practitioners to join our friendly expanding
General Practice in Upper Gornal and Tipton for up to 30 hours per week over 3-4 days.
The post holder will be an
experienced Nurse Practitioner and independent prescriber who, acting within their professional boundaries will
provide care for the presenting patient from initial history taking, clinical
assessment, diagnosis, treatment and evaluation of their care. They will
demonstrate safe, clinical decision-making and expert care for patients within
the general practice.
Main duties of the job
We expect that you will be able to work independently and manage your
own workload. You will need excellent communication skills, able to meet
targets and to be able to liaise with patients, primary care staff, community
pharmacists and other partners.
Must have experience of clinical triage, home visits and be an independent prescriber.
Carries out clinical nursing practice
at an advanced level using expert clinical skills, critical judgements &
knowledge to deliver appropriate and safe care.
Assess, diagnose and treat patients
in practice premises or their own home who require medical attention and/or
refer to appropriate agency.
You will be part of a dynamic and innovative team who value
teamwork, professional practice, mutual support and a friendly working
environment.
You will have 1 ward
round per week to a nursing home either virtually or F2F to support residential
patients

years, (most

MANAGEMENT OF PATIENT HEALTH AND ILLNESS
1.1.
To deliver a high standard of patient care as Advanced Nurse Practitioner in
general practice, using advanced autonomous clinical skills, and a broad and
in-depth theoretical knowledge base.
1.2.
Provide a first point of contact within the Practice for patients aged five
years and upwards presenting with undifferentiated, undiagnosed problems,
making use of skills in history taking, physical examination, problem-solving

investigations and interpret findings/reports.

Promote health and well-being through the use of health promotion, health
