Job summary

Todmorden Group Practice is a thriving and supportive practice based in the semi-rural Victorian market town of Todmorden.

The location is scenic with great road and train links into Manchester and Leeds. We are an active training practice with high QOF achievement and aim to provide a high standard of care.

Main duties of the job

We are looking for a highly motivated and enthusiastic Practice Nurse. We are family friendly and happy to discuss flexible arrangements.

The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population.

A focus on the clinical delivery of evidence based practice for patients with long- term conditions and management and preventative nursing interventions to all patients.

Work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing support and direction for the nursing team.

About us

The team consists of 4 GP Partners, 6 Salaried GPs and a nursing team of 9.

We have excellent links with the Upper Calder Valley PCN and engage with the ARRS roles, including, First Contact Physiotherapists, Visiting clinicians, Team of Pharmacist, Social Prescribers, Care Co-ordinators, and an Ageing Well Team. We are truly an MDT.

Clinical Practice

Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition

Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including using non-drug-based treatment methods, in line with national and local policies and practice needs

Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national and practice protocols

Work with patients in order to support adherence to prescribed treatments

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Support patients to adopt health promotion strategies that promote them to live healthily, and apply principles of self-care

Support and manage health needs of women presenting for family planning and cervical cytology consultations

Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health as required

Implement and participate in vaccination and immunisation programmes for both adults and children

Advise, support and administer vaccinations for patients travelling abroad

Promote and deliver evidence-based care for patients presenting with aural conditions

Meet the needs of patients presenting for opportunistic wound care

Organisation

Take responsibility for own development, learning and performance including participating in clinical supervision and acting as a positive role model

Clinical and operations support to the PN and HCA team, maximising the efficiency and effectiveness of the team and ensuring the delivery of a high quality service for patients

Organise work and delegate tasks where appropriate in ensuring safe practice.

Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice

Work with other senior staff to ensure the achievement of Practice aims and objectives and the efficient and effective running of the nursing provision.

Contribute towards setting policy for the clinical service and nursing team, ensuring the achievement of targets (e.g. under the Quality and Outcomes framework) and improve uptake of screening and vaccination programmes.

Assess, implement, monitor, and ensure the quality of enhanced services delivered.

Collate, analyse and present data and information.

Influence, plan, develop and implement specific projects and new nursing services required.

Ensure that nursing guidelines, policies and other documents (including PGDs) are up-to-date and that adequate and effective recall systems are in place, as appropriate.

Identify the nursing requirement to meet the practice population need, assessing the impact and implementation of skill-mix in the delivery of care and developing new services and increasing skill-mix as appropriate.
