Job summary

We are set in a beautiful semi-rural Victorian market town in the Pennines, with excellent transport links between Leeds and Manchester.

We are a forward thinking GMS training Practice with approximately 13500 patients and aim to provide high quality care for our traditional community. We hold a CQC rating of GOOD.

We are located within impressive, spacious modern premises with a wide array of clinical facilities and Multi-disciplinary team onsite.

We are part of Upper Calder Valley PCN and collectively we offer a lot of additional services to help us support our population holistically.

We are currently looking to recruit an Practice Nurse due to a significant expansion of our practice patient list.

Main duties of the job

Job Purpose

To act as an autonomous Nurse working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients. Working primarily with patients withinCare and Nursing homes.

Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.

About us

Close knit, friendly team of 4 partners, 4 salaried GP and a fantastic nursing team of 7, supported by the Patient Service Advisors and Management team.

We work closely with our PCN colleagues including, Clinical Pharmacist, Pharmacy technicians, Care Co-ordinators, Social Prescribers, First Contact Physio's and Podiatrist.

We have a strong track record of staff retention.

Job responsibilities

CLINICAL PRACTICE

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition.

Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including using non-drug-based treatment methods, in line with national and local policies and practice needs.

Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national and practice protocols.

Work with patients in order to support adherence to prescribed treatments.

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health as required.

Promote and deliver evidence-based care for patients presenting with aural conditions.

ORGANISATION

Take responsibility for own development, learning and performance including participating in clinical supervision and acting as a positive role model.

Clinical and operations support to the PN and HCA team, maximising the efficiency and effectiveness of the team and ensuring the delivery of a high quality service for patients.

Organise work and delegate tasks where appropriate in ensuring safe practice.

Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice.

Work with other staff to ensure the achievement of Practice aims and objectives and the efficient and effective running of the nursing provision.

Contribute towards setting policy for the clinical service and nursing team, ensuring the achievement of targets (e.g. under the Quality and Outcomes framework) and improve uptake of screening and vaccination programmes.

Assess, implement, monitor, and ensure the quality of enhanced services delivered.

Collate, analyse and present data and information.

Influence, plan, develop and implement specific projects and new nursing services required.

Ensure that nursing guidelines, policies and other documents (including PGDs) are up-to-date and that adequate and effective recall systems are in place, as appropriate.

Mentor individual student Nurses on placement liaising with the Education coordinator
