Job summary
We have a fantastic opportunity for an experienced practice nurse to join our nursing team, working across Oxford Terrace and Rawling Road practices. This position offers the opportunity to work alongside a professional, multi-skilled and friendly nursing team supported by a nurse lead and strong partnership.
Opportunities to develop clinically, within the practice are encouraged and supported. We are a teaching practice with a great team of permanent GP's, frailty nurse practitioners, practice nurses, nursing associates and healthcare assistants, offering a reasonable and well organised workload structure.
This is a part-time role of 32 hours per week. Interviews are scheduled for Thursday 12th March 2026.
Main duties of the job
You will be responsible for the delivery of day to day practice nursing services to the practice population.
Supported by a senior clinical team within the practice,you will deliver care within the boundaries of your role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities.You will work collaboratively with the general practice team to meet the needs of the patients and to refer into complementary services as required; supporting the delivery of policy and procedures, and providing nurse leadership as required.
The post holder may, in the future, be required to attend local primary care network meetings and support other developing activities in the network.

Implement and evaluate individual treatment plans for patients with a known long-term condition
Identify and manage as appropriate, treatment plans for patients at risk of developing a long-term condition and support health prevention programmes
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care
Deliver opportunistic health promotion using opportunities such as new-patient and NHS Health Checks.
Provide sensible and up-to-date information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions, seeking clinical support as appropriate
Assess and care for patients presenting with uncomplicated wounds
Support and advise women requesting information relating to family planning needs
Support and manage health needs of women presenting for cervical cytology consultations
Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health and the Mental Capacity Act (MCA)
Implement and participate in vaccination and immunisation programmes for both adults and children
Provide paediatric phlebotomy services where competent or to agree to train towards competency within six months of commencement of duties
Advise, support and, where appropriate, administer vaccinations for patients travelling abroad
