Job summary
Birtley and Central Gateshead PCN are looking for a highly motivated and enthusiastic pharmacist to join our existing team of pharmacists and pharmacy technician.
This opportunity will give the successful candidate the opportunity to
work with an experienced pharmacy team in a diverse, community focused
practice based over two sites.

Main duties of the job
The
post holder will act within their professional boundaries, supporting and
working alongside a multi-disciplinary team in a Primary Care network. In this
role, they will be supported by a named GP who will develop, manage and mentor
them. They will either have completed or willing to complete CPPE pathway.
The
post holder will provide primary support to general practice staff with regards
to prescription and medication queries. They will help support the repeat
prescription system and medicines reconciliation on transfer of care and
systems for safer prescribing, providing expertise in clinical medicines advice
while addressing both public and social care needs of patients across the PCN.
The post
holder will ensure that PCN practices integrate with community and hospital
pharmacy to help utilise skill mix, improve patient outcomes, ensure better
access to healthcare and help manage workload. The role is pivotal to improving
the quality of care and operational efficiencies so requires motivation and
passion to deliver excellent service within general practice.

and other local providers of healthcare;

plan and organise the post holders own workload, including audit and project
work, and training sessions for members of the practice team;

record personally generated information and maintain a database of information

by the post holder, reference notes relating to clinical/technical information,
etc the database includes maintaining
up-to-date, detailed records of all work done in the practices for which the
