Job summary
The post holder is
responsible for the delivery of practice nursing services to the practice
population. Supported by the Nurse Manager, they will deliver care within the
boundaries of their role, working within practice protocols, focusing upon
supporting patients to be healthy, monitoring of long-term conditions, health
prevention and screening activities. The
duties will include all tasks normally undertaken by an experienced RN and in
addition any roles agreed between the nurse and the doctors as appropriate,
having regard to current training. They will work collaboratively
with the general practice team to meet the needs of patients, supporting the
delivery of practice policies and procedures and will also need to have an
understanding of the public health profile and population needs in order to be
proactive in ensuring services are met.
Main duties of the job
- Carry out annual reviews and management for all long-term conditions, maintaining the annual recall system
- Diabetic Insulin Initiation desirable or willing to undertake qualification.
- Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
- Following agreed clinical protocols with referral to senior nurses or GPs as appropriate assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
- Deliver opportunistic health promotion using opportunities such as new-patient medicals
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions
- Cytology- Support and manage health needs of women presenting for cervical cytology consultations.
- Family Planning - Support and advise women requesting information relating to family planning needs and contraception.
- Travel Clinic - Offer travel health advice by providing comprehensive advice to patients prior to travel including; vaccinations and medicines, safe sex, food hygiene, sun protection.
- Routine immunisations/Childhood immunisations - Implement and participate in vaccination and immunisation programmes for both adults and children.

We are an established Training Practice with 6 Partners, 6 Salaried GPs, 5 Practice Nurses, 1 Nursing Associate, 4 Trainee Nurse Associates, 3 Healthcare Assistants and 1 Advanced Nurse Practitioner with a practice population of just over 15,000 and growing.
Working as part of a progressive and effective primary care team, the successful applicant should have at least a minimum of 1 year post-registration experience. Be highly organised, motivated, and computer literate and be able to demonstrate continuing professional development. Showing a commitment to promoting nursing in primary care is essential for this exciting post.
