Job summary
The Grange practice is looking to recruit an additional member of the clinical team to provide care to our patients that are either in a care/residential home, support living, housebound or becoming more frail, therefore, making it more difficult for them to attend appointments are the surgery.
You will be directly employed by the surgery - please note this is not a district nurse position.
This is a real opportunity to work autonomously whilst still being part of a team, utilising all your skills and knowledge, with the opportunity to develop new skills including prescribing and chronic disease management.
For this role we are looking to employ either a registered general nurse, nurse associate or prescribing nurse. We do not use Agenda for Change but remunerate based on the skills of the individual.
Main duties of the job
Coming from a nursing background, you will be a skilled in holistic patient centred care, undertaking proactive as well as acute care related visits.
You will work out of the Grange Practice visiting patients at their residence either on your own or on a joint visit. Some of your visits will be planned by yourself and others requested by another member of the practice team.
Duties will be in line with your competencies, but examples for proactive care would be Diabetic foot checks, ECGs for heart failure review, blood pressure measurements, venepuncture, doppler, dementia reviews and ReSPECT/Advanced care planning.
For acute visits, you may be asked to perform basic observations such as blood pressure, temperature, urine dipping, ear syringing, assessment of wound, and if qualified and competent - to treat minor illness.
About us
The Grange Practice is a Partnership led practice with 3 GP partners and a non-clinical partner.
We currently employ 5 Salaried GPs, 1 advance nurse practitioner, 2 nurse practitioners (including one community based), 2 advance clinical practitioners, 2 clinical pharmacists, 1 pharmacy technician, 1 first contact physiotherapist, 2 practice nurses, 2 nurse associates, 2 HCAs, 1 Physician Assistant/Dietician and various different administrative staff.
We are a training practice and currently have 3 GPs in training working within the practice. We also offer placements for student nurses, physician assistants and medical students.
We are part of Ramsgate Primary Care Network and strongly believe in collaborative working to enhance patient care.
We are working on a new surgery development which will enable the practice to employ more clinical staff and develop more clinical services as well as offering space to other health care and voluntary sector colleagues.
